Poliklinik şartlarında hastaların tıbbi tedavileri ve ilaçları reddetme nedenleri ile bu durumlara yönelik geliştirilmesi gereken klinik yaklaşımlar, etik ve yasal çerçevede ele alınmaktadır. Araştırmalar, hastaların önemli bir kısmının tedaviye tam uyum göstermediğini ve çeşitli dinsel, kişisel veya zihinsel gerekçelerle tedaviyi reddettiğini ortaya koymaktadır. Bu ret kararlarının arkasında kan transfüzyonunu kabul etmeyen Yehova Şahitleri, acı çekmeyi manevi gelişim gören Budistler veya oruç döneminde oral alımı kesen Müslümanlar gibi dini inançlar yer alabilmektedir. Kanser hastalarında yoğun yan etki korkusu ve yaşam sonu sürecini kontrol etme arzusu öne çıkarken; semptom hissetmeme, HIV tedavisindeki damgalanma korkusu veya aşı karşıtlığı gibi yetersiz/yanlış bilgiler de ret kararlarını tetiklemektedir. Hayvansal ürün içeren ilaçlar vegan/vejetaryen hastalarca reddedilirken; şizofreni ve bipolar bozukluk gibi durumlarda ise düşük içgörü tedavi uyumunu bozmaktadır. Hekimlerin hastayı zorlaması etik dışı olup, bilgilendirilmiş onam prensibine sadık kalarak ret nedenini belirlemesi, alternatif seçenekler sunması, hastanın karar verme yetisini testlerle değerlendirmesi ve tüm süreci Türk Medeni Kanunu gibi yasal zeminlere uygun şekilde ayrıntılı olarak kayıt altına alması önerilmektedir.

The medical treatment and medication refusal reasons of patients in outpatient clinics and the clinical approaches to be developed for these situations are discussed within ethical and legal frameworks. Research reveals that a significant portion of patients do not fully adhere to treatment and refuse it due to various religious, personal, or mental reasons. Behind these refusal decisions, religious beliefs can play a role, such as Jehovah's Witnesses who reject blood transfusions, Buddhists who view suffering as spiritual growth, or Muslims who cease oral intake during fasting periods. While intense fear of side effects and the desire to control the end-of-life process come to the fore in cancer patients; insufficient/incorrect information such as not feeling symptoms, fear of stigma in HIV treatment, or vaccine hesitancy also triggers refusal decisions. Medications containing animal products are rejected by vegan/vegetarian patients, whereas in conditions like schizophrenia and bipolar disorder, low insight impairs treatment compliance. It is unethical for physicians to force the patient; instead, adhering to the principle of informed consent, they are recommended to determine the reason for refusal, offer alternative options, evaluate the patient's decision-making capacity with tests, and meticulously document the entire process in accordance with legal grounds such as the Turkish Civil Code.
