Poliklinikte Medikal Probleme Neden Olan Bağımlılıklarda Motivasyonel Yaklaşım

Özet

Bağımlılığa sahip hastaların tıbbi yönetiminde karşılaşılan zorluklar karşısında motivasyonel yaklaşımın önemini ele alan bu çalışma, bağımlılığın bir irade eksikliğinden ziyade nörobiyolojik temelleri olan psikiyatrik bir hastalık olduğunu vurgulamaktadır. DSM-5 kapsamında on ayrı madde kümesi tanımlanmakta olup klinik açıdan işlevsellikte gerilemeye yol açan kronik örüntüler, tolerans ve yoksunluk belirtileriyle kendini gösterir. Bu hastaların preventif sağlık hizmetlerinden mahrum kalması tıbbi ek maliyetler doğururken; kardiyak, hepatik ve enfeksiyöz gibi geniş alanlarda ciddi riskler oluşturmaktadır. Kısa müdahale görüşmelerinin değişimi başlatmadaki rolü büyüktür; zira paternalistik ve suçlayıcı yaklaşımlar yerine danışanın kendini güvende hissettiği işbirlikçi bir ortam içsel motivasyonu artırır. DiClemente ve Prochaska tarafından geliştirilen Transteorik Model; farkındalık öncesi, farkındalık, hazırlanma, eylem ve sürdürüm evreleriyle hastadaki dinamik süreci açıklar. Poliklinik koşullarında hekimlerin otoriter konumdan kaçınarak danışanın özerkliğini, amaçlarını desteklemesi ve direnç karşısında esnek olmaları önerilir. Sonuç olarak amaç tek görüşmede tüm sorunları çözmek değil, hastanın özyeterliliğini destekleyen kısa müdahalelerle gelecek için açık kapı bırakmaktır.

This study, which addresses the importance of the motivational approach in the face of difficulties encountered in the medical management of patients with addiction, emphasizes that addiction is a psychiatric illness with neurobiological foundations rather than a lack of willpower. Ten distinct substance groups are defined under DSM-5, manifesting as chronic patterns, tolerance, and withdrawal symptoms that cause a decline in clinical functionality. While the deprivation of preventive health services among these patients creates additional medical costs, it poses serious risks across broad areas such as cardiac, hepatic, and infectious diseases. The role of brief intervention interviews in initiating change is significant; because instead of paternalistic and accusatory approaches, a collaborative environment where the client feels safe increases intrinsic motivation. The Transtheoretical Model, developed by DiClemente and Prochaska, explains the dynamic process in the patient through the stages of precontemplation, contemplation, preparation, action, and maintenance. In outpatient clinic conditions, physicians are advised to avoid an authoritarian stance, support the client's autonomy and goals, and remain flexible when encountering resistance. Consequently, the goal is not to solve all problems in a single session, but to leave an open door for the future through brief interventions that support the patient's self-efficacy.
