Psikiyatride Sık Kullanılan İlaçlar ve Sık Görülen İlaç Etkileşimleri

Özet

Ruhsal rahatsızlıkların tedavisinde kullanılan antidepresanlar, duygudurum dengeleyiciler, psikostimülanlar, benzodiazepinler ve antipsikotikler gibi temel psikofarmakolojik ajanlar ile bunların yan etkileri ve ilaç etkileşimleri psikiyatrik sağaltımda kritik bir öneme sahiptir. Antidepresanlar geniş bir yelpazede tercih edilirken; trisiklik antidepresanlar (TCA), selektif serotonin gerialım inhibitörleri (SGİ) ve serotonin noradrenalin gerialım inhibitörleri (SNGİ) gibi farklı grupların kendilerine has yan etki profilleri ve sitokrom P450 enzimi üzerinden farmakokinetik etkileşim riskleri mevcuttur. Bipolar bozuklukta kullanılan lityum, valproat ve karbamazepin gibi duygudurum dengeleyicilerin ise terapötik aralıklarının dar olması nedeniyle serum düzeyleri ve organ fonksiyonları düzenli takip edilmelidir; nitekim lityum toksisitesi veya serotonerjik ajanların kombinasyonuyla tetiklenen serotonin sendromu ölümcül sonuçlar doğurabilir. GABA nörotransmisyonunu güçlendiren benzodiazepinler kısa süreli anksiyolitik ve sedatif etkiler sunarken, uzun süreli kullanımda bağımlılık ve bilişsel bozulma riski taşır. Antipsikotikler ise şizofreni ve manik epizodlar gibi psikotik belirtilerin sağaltımında birinci ve ikinci kuşak olarak sınıflandırılır; ikinci kuşak ajanlar daha az ekstrapiramidal yan etki gösterse de metabolik profili olumsuz etkileyebilir ve tüm antipsikotikler potansiyel olarak ölümcül nöroleptik malign sendrom (NMS) riskini barındırır.

Psychopharmacological agents such as antidepressants, mood stabilizers, psychostimulants, benzodiazepines, and antipsychotics used in the treatment of mental disorders, along with their side effects and drug interactions, hold a critical role in psychiatric management. While antidepressants are preferred across a wide spectrum, different groups like tricyclic antidepressants (TCAs), selective serotonin reuptake inhibitors (SSRIs), and serotonin-norepinephrine reuptake inhibitors (SNRIs) possess unique side effect profiles and pharmacokinetic interaction risks via cytochrome P450 enzymes. Mood stabilizers used in bipolar disorder, including lithium, valproate, and carbamazepine, require regular monitoring of serum levels and organ functions due to their narrow therapeutic windows; indeed, lithium toxicity or serotonin syndrome triggered by the combination of serotonergic agents can lead to fatal outcomes. Benzodiazepines, which enhance GABA neurotransmission, offer short-term anxiolytic and sedative effects but carry risks of dependence and cognitive impairment in long-term use. Antipsychotics are classified into first and second generations for the management of psychotic symptoms in conditions such as schizophrenia and manic episodes; although second-generation agents exhibit fewer extrapyramidal side effects, they can negatively impact the metabolic profile, and all antipsychotics carry the risk of the potentially fatal neuroleptic malignant syndrome (NMS).
