Poliklinik Koşullarında Uykusuzluğa Yaklaşım ve Pratik Öneriler

Poliklinik koşullarında uykusuzluk bozukluğunun (insomnia) tanımı, sınıflandırılması, etiyolojisi, klinik değerlendirmesi ve tedavisine yönelik kapsamlı pratik öneriler sunan makalede, uykunun genel sağlık üzerindeki hayati işlevleri ve tedavi edilmeyen uykusuzluğun küresel bir tehdit oluşturduğu vurgulanmaktadır. Genel nüfusun %10-20'sini etkileyen bu rahatsızlık için güncel ICSD-3 ve DSM-5 tanı sistemleri, birincil-ikincil ayrımını kaldırarak uykusuzluğu kendi içinde kronik bir bozukluk olarak ele almakta; tanı için haftada en az 3 kez ve en az 3 ay süre koşulu aramaktadır. Yorgunluk, dikkat dağınıklığı ve işlevsellik kaybına yol açan bozukluğun etiyolojisinde; Spielman'ın Üç Faktörlü Modeli, Nörobilişsel ve Hiperarousal modelleri öne çıkmakta ve uykusuzluğun psikofizyolojik yatkınlık, stresörler ve uyumsuz davranışlar zinciriyle kronikleştiği belirtilmektedir. Değerlendirmede detaylı klinik öykü ve uyku günlükleri kritik rol oynar. Farmakolojik olmayan yaklaşımlar (uyku hijyeni, uyaran kontrolü, gevşeme yöntemleri, paradoksal niyetlenme) tedavinin birinci basamağı ve en güvenli seçeneğidir. İlaç tedavileri ise tolerans ve bağımlılık riskleri nedeniyle hastaya özgü, en düşük dozda ve 2-4 hafta gibi kısa süreli planlanmalıdır.

This article presents comprehensive practical recommendations on the definition, classification, etiology, clinical evaluation, and treatment of insomnia in outpatient settings, emphasizing that sleep is a vital biological function for health and untreated insomnia poses a global threat. Affecting 10-20% of the general population, current ICSD-3 and DSM-5 diagnostic systems eliminate the primary-secondary distinction, treating insomnia as a chronic disorder in its own right, requiring a frequency of at least 3 times a week for at least 3 months for diagnosis. The condition, which causes fatigue, attention impairment, and loss of functionality, is explained through the Three-Factor Model of Spielman, Neurocognitive, and Hyperarousal models, indicating that insomnia becomes chronic via a chain of psychophysiological predisposition, stressors, and maladaptive behaviors. Detailed clinical history and sleep diaries play a critical role in evaluation. Non-pharmacological approaches (sleep hygiene, stimulus control, relaxation techniques, paradoxical intention) are the first-line and safest treatment options. Pharmacological treatments should be planned case-specifically at the lowest effective dose for a short duration of 2-4 weeks due to tolerance and dependence risks.
