Polikilinikte Kullanılabilecek Bazı Motivasyonel Görüşme Teknikleri
Özet
Motivasyonel görüşme, bireylerin kendi motivasyonlarını ve değişime olan bağlılıklarını güçlendirmeyi amaçlayan, işbirliğine dayalı, kabul ve şefkat atmosferi barındıran terapötik ve yönlendirici bir rehberlik tarzıdır. Geleneksel ve otoriter tavsiye verme yöntemlerine kıyasla davranış değişikliği sağlamada daha yüksek etkinlik gösteren bu yaklaşım; sigara kullanımı, obezite, kronik hastalık yönetimi ve tedavi uyumu gibi birçok klinik alanda ve birinci basamak sağlık hizmetlerinde yaygın şekilde uygulanmaktadır. Motivasyonel görüşme, ezberlenecek katı bir protokolden ziyade hekimin zamanla kazanacağı dinamik becerileri temsil eder. Süreç boyunca hastanın değişim ile mevcut durumu sürdürme isteği arasında yaşadığı ve "ambivalans" olarak adlandırılan çelişkiler olağan kabul edilir. Hekim, "düzeltme refleksi" ile hastaya ne yapacağını söylemek yerine; işbirliği, kabul, şefkat ve hastanın içindeki potansiyeli açığa çıkarma ilkelerine dayanan motivasyonel görüşmenin ruhunu benimsemelidir. Bu süreç; katılım sağlama, odaklanma, açığa çıkarma ve planlama olmak üzere birbirinin üzerine inşa edilen dört temel basamaktan oluşur. Sonuç olarak, kanıta dayalı tıbba dayanan bu teknikler, hekimlerin sınırlarının farkında olarak mesleki tatminlerini artırmalarını ve hastalarıyla uyum içinde kalıcı davranış değişiklikleri elde etmelerini sağlar.
Motivational interviewing is a collaborative, accepting, and compassionate therapeutic guiding style aimed at strengthening individuals' own motivation and commitment to change. Demonstrating higher efficacy in behavioral changes compared to traditional authoritarian advice, this approach is widely utilized in primary care and various clinical areas such as smoking, obesity, chronic disease management, and treatment adherence. Rather than a rigid protocol, it represents dynamic skills acquired by physicians over time. The internal conflict experienced between the desire to change and maintaining the current status, termed "ambivalence," is considered natural. Instead of utilizing a "righting reflex" to dictate actions, clinicians must embody the spirit of motivational interviewing based on partnership, acceptance, compassion, and evocation. This process structurally comprises four consecutive stages: engaging, focusing, evoking, and planning. Ultimately, these evidence-based techniques enable physicians to enhance professional satisfaction while achieving harmonious, sustainable behavioral modifications.
