Kalp Damar Cerrahisi Anestezisinde Kullanılan İnovatif Gelişmeler

Özet

Bu çalışma, kalp damar cerrahisi anestezisindeki inovatif yaklaşımları ve teknolojik gelişmeleri kapsamlı bir şekilde ele almaktadır. Başarılı bir operasyon süreci için preoperatif, intraoperatif ve postoperatif dönemlerin titizlikle dizayn edilmesi gerektiğini vurgulayan metin, temel olarak dört ana başlığa odaklanmaktadır. Preoperatif dönemde "kırılganlık indeksi" skorlamasının önemi belirtilerek; yaşlı hastalarda azalan fizyolojik rezervlerin mortalite ve morbidite riskini artırdığı, bu nedenle Fried ve Edmonton gibi testlerin prognostik değer taşıdığı ifade edilmektedir. Miyokard koruması başlığında ise artık sürecin sadece ameliyat anıyla sınırlı kalmadığı; kök hücre nakli, miRNA uygulamaları ve mitokondriyal fonksiyonların düzenlenmesi gibi ileri yöntemlerin kalp dokusunu korumada kritik rol oynadığı aktarılmaktadır. İlaç yönetiminde Omecamtiv mecarbil ve Istaroksim gibi yeni inotropik ajanlar ile volatil anesteziklerin kardiyoprotektif etkileri incelenmektedir. Cihazlar ve kanama yönetimi kısmında ise böbrek hasarını öngören Nephrocheck, serebral oksijenasyonu izleyen NIRS ve kanama kontrolünde standart testlerin ötesine geçen ROTEM/TEG algoritmaları üzerinde durulmaktadır. Sonuç olarak, teknolojik ürünlerin ve güncel protokollerin kullanımı, kalp cerrahisinde hasta güvenliğini ve hizmet kalitesini artıran temel unsurlar olarak sunulmaktadır.

This study comprehensively examines innovative approaches and technological advancements in cardiovascular surgery anesthesia. Emphasizing that preoperative, intraoperative, and postoperative periods must be meticulously designed for a successful operation, the text focuses on four main categories. In the preoperative period, the importance of "frailty index" scoring is highlighted, noting that decreased physiological reserves in elderly patients increase mortality and morbidity risks; thus, tests like Fried and Edmonton carry prognostic value. Under myocardial protection, it is stated that the process is no longer limited to the moment of surgery; advanced methods such as stem cell transplantation, miRNA applications, and the regulation of mitochondrial functions play a critical role in protecting heart tissue. In pharmacological management, new inotropic agents like Omecamtiv mecarbil and Istaroxime, along with the cardioprotective effects of volatile anesthetics, are analyzed. In the section on devices and bleeding management, Nephrocheck for predicting renal injury, NIRS for monitoring cerebral oxygenation, and ROTEM/TEG algorithms that go beyond standard tests for bleeding control are emphasized. In conclusion, the use of technological products and updated protocols are presented as fundamental elements that enhance patient safety and service quality in cardiac surgery.
