Anestezi Alanında İnovatif Kavramlar: “İkinci Mağdur (Kurban) Olgusu”

Tıp mesleğinde kusursuzluk beklentisi hakim olsa da, sağlık hizmetlerinin insan odaklı ve dinamik doğası tıbbi hataları kaçınılmaz kılmaktadır. Olumsuz bir tıbbi olayda, zarar gören hasta "birinci mağdur" iken, bu olay nedeniyle derin duygusal travma yaşayan sağlık çalışanı "ikinci mağdur" olarak tanımlanır. Araştırmalar, sağlık çalışanlarının yaklaşık %50'sinin kariyerleri boyunca en az bir kez bu durumu deneyimlediğini göstermektedir. Anestezi ve reanimasyon hekimleri, yoğun iş temposu, yüksek mortalite riski taşıyan ameliyatlar ve pandemi gibi stresli süreçler nedeniyle İMO açısından yüksek risk altındadır. İkinci mağdurlar; utanç, suçluluk, anksiyete, depresyon ve hatta intihar düşüncesi gibi ağır psikolojik sorunlar yaşayabilirler. Bu durum sadece bireysel sağlığı değil, aynı zamanda odaklanma güçlüğü ve özgüven kaybı yaratarak hasta güvenliğini de tehdit eder. Kurumsal destek sistemlerinin ve akran desteğinin yetersizliği, bu travmanın atlatılmasını zorlaştırmaktadır. Sonuç olarak, Türkiye'de henüz yeterince tanınmayan İMO konusunda farkındalık yaratılması ve anestezistler için yapılandırılmış destek mekanizmalarının kurulması kritik bir ihtiyaçtır.

Although perfection is expected in the medical profession, the human-centered and dynamic nature of healthcare makes medical errors inevitable. In an adverse medical event, while the harmed patient is the "first victim," the healthcare professional who is deeply traumatized by the event is defined as the "second victim". Research indicates that approximately 50% of healthcare providers experience this phenomenon at least once in their careers. Anesthesiology and reanimation specialists are at high risk for the second victim phenomenon (SVP) due to intense workloads, surgeries with high mortality risks, and stressful periods like pandemics. Second victims may experience severe psychological issues such as shame, guilt, anxiety, depression, and even suicidal ideation. This situation threatens not only individual health but also patient safety by causing difficulty in concentration and loss of self-confidence. The inadequacy of institutional support systems and peer support makes it difficult to overcome this trauma. Consequently, raising awareness about SVP, which is not yet sufficiently recognized in Turkey, and establishing structured support mechanisms for anesthesiologists is a critical necessity.
