Spinal Enfeksiyonların Nöroşirürjik Acil Yönetim
Özet
Spinal enfeksiyonlar; vertebra, disk, epidural alan ve yumuşak dokuları etkileyen, sıklıkla spondilodiskit olarak adlandırılan ciddi klinik durumlardır. Bu enfeksiyonlar postoperatif bir komplikasyon olarak veya spontan şekilde gelişebilirken, en yaygın etken %30-80 oranında S.aureus olarak saptanmıştır. Yayılım genellikle hematojen yolla gerçekleşmekte olup, en sık lomber bölge etkilenmektedir. Klinik tablo; nonspesifik sırt ağrısı ve ateşten, motor kayıplar ve paralizi gibi ağır nörolojik defisitlere kadar geniş bir yelpazede seyredebilir. Tanı sürecinde CRP ve sedimantasyon hızı gibi laboratuvar belirteçleri yol gösterici olsa da Manyetik Rezonans Görüntüleme (MRG) altın standart tetkiktir. Tedavi planı; patojene özgü 6-8 haftalık antibiyotik kullanımı ve immobilizasyonu kapsar. Ancak nörolojik defisit, yapısal instabilite, medikal tedaviye direnç veya belirgin kemik destrüksiyonu gibi durumlarda cerrahi müdahale zorunlu hale gelmektedir.
Spinal infections are serious clinical conditions affecting the vertebrae, discs, epidural space, and soft tissues, frequently referred to as spondylodiscitis. These infections can emerge as postoperative complications or spontaneously, with S.aureus identified as the most common pathogen in 30-80% of cases. Dissemination generally occurs through hematogenous routes, most frequently affecting the lumbar region. The clinical presentation ranges from non-specific back pain and fever to severe neurological deficits such as motor loss and paralysis. While laboratory markers like CRP and erythrocyte sedimentation rate provide guidance during diagnosis, Magnetic Resonance Imaging (MRI) remains the gold standard. The treatment plan involves 6-8 weeks of pathogen-specific antibiotic therapy and immobilization. However, surgical intervention becomes mandatory in cases involving neurological deficits, structural instability, resistance to medical treatment, or significant bone destruction.
