Sarcopenia is a major geriatric health problem characterized by a progressive loss of skeletal muscle mass, muscle strength, and physical performance, particularly associated with aging. With a prevalence ranging between 1% and 29% in the general population, it reaches up to one-third among frail elderly individuals living in nursing homes. Lack of physical activity, inadequate low-protein nutrition, chronic inflammation, and age-related physiological decline contribute to its development. Guided by EWGSOP2, the diagnostic process evaluates handgrip strength, muscle mass index, and gait speed, utilizing techniques such as DEXA, BIA, the Short Physical Performance Battery, the Timed Up and Go test, and the SARC-F questionnaire. Exercise and nutritional interventions play a key role in the primary, secondary, and tertiary prevention and treatment of sarcopenia. Nutritionally, the Mediterranean diet, daily high-quality protein of 1.0-1.5 g/kg, leucine, essential amino acids, and vitamin D supplementation are recommended. In terms of exercise, resistance training (RE), aerobic exercise training (AET), and high-intensity interval training (HIT) preserve muscle mass and functional capacity by enhancing mitochondrial biogenesis, protein synthesis, and insulin sensitivity. For elderly individuals unable to engage in regular exercise, whole-body vibration (WBV) and whole-body electromyostimulation (WB-EMS) offer alternative therapeutic options.
