Bebek ve Çocuklarda İşitmenin Değerlendirilmesi
Özet
İşitme kaybı, gelişmekte olan ülkelerde yenidoğanlarda sık görülen konjenital anomalilerden biri olup erken dönemde tanılanıp müdahale edilmediğinde çocukların dil, bilişsel, sosyal ve akademik gelişimlerini olumsuz yönde etkilemektedir. Konjenital işitme kayıplarının erken tespiti amacıyla geliştirilen Ulusal Yenidoğan İşitme Tarama Programları (UYİTP), bebeklerin ilk 1 ayında taramayı, 3 ayında tanıyı ve 6 ayında ise müdahale hizmetlerine erişimini hedeflemektedir. Bu programlarda objektif ve fizyolojik ölçümler sağlayan Otomatik-Anlık Uyarılmış Otoakustik Emisyon (A-TEOAE) ve ülkemizde temel tarama testi olarak uygulanan Otomatik İşitsel Beyinsapı Cevabı (A-ABR) yöntemleri kullanılmaktadır. Bebeklerin kronolojik yaşlarından ziyade gelişimsel ve bilişsel seviyelerine uygun olarak seçilen Davranım Gözlem (BOA), Görsel Pekiştireç (VRA) ve Şartlanmış Oyun Odyometrisi (CPA) gibi davranışsal testler pediatrik grupta işitsel hassasiyetin değerlendirilmesinde altın standart kabul edilmektedir. Çocuklarda işitsel mekanizmanın karmaşık yapısı nedeniyle tek bir test yerine çoklu test bataryası yaklaşımı benimsenmekte; orta kulak fonksiyonlarını inceleyen Timpanometri ve Akustik Refleks ölçümlerinin yanı sıra koklear dış tüy hücre aktivitesini değerlendiren EOAE ile işitme siniri ve beyinsapı nöral yollarını analiz eden objektif ABR ve ASSR gibi elektro/fizyolojik testler birlikte kullanılmaktadır.
Hearing loss is one of the most common congenital anomalies seen in newborns in developing countries, and when not diagnosed and intervened in the early period, it negatively affects children's language, cognitive, social, and academic development. The National Newborn Hearing Screening Programs (NNHSP), developed for the early detection of congenital hearing loss, aim for screening within the first month of infants, diagnosis within three months, and access to intervention services within six months. Automated-Transient Evoked Otoacoustic Emissions (A-TEOAE) and Automated Auditory Brainstem Response (A-ABR), which is implemented as the primary screening test in our country, are utilized in these programs providing objective and physiological measurements. Behavioral tests selected in accordance with the developmental and cognitive levels of infants rather than their chronological ages, such as Behavioral Observation (BOA), Visual Reinforcement (VRA), and Conditioned Play Audiometry (CPA), are considered the gold standard in evaluating auditory sensitivity in the pediatric group. Due to the complex structure of the auditory mechanism in children, a cross-check test battery approach is adopted instead of a single test; along with Tympanometry and Acoustic Reflex measurements examining middle ear functions, electro/physiological tests such as EOAE evaluating cochlear outer hair cell activity, and objective ABR and ASSR analyzing the auditory nerve and brainstem neural pathways are utilized together.
