Özet

Büyüme-gelişim (matürasyon) ve kemik yaşı tayini; ortodontide teşhis, tedavi planlaması ve tedavi zamanlamasının belirlenmesinde kritik bir rol oynamaktadır. Kronolojik yaş, bireysel biyolojik gelişimle her zaman paralel gitmediğinden tek başına yetersiz kalmakta ve bu nedenle biyolojik gelişimi yansıtan kemik ve diş yaşları gibi parametreler esas alınmaktadır. Diş hekimliğinde kemik yaşı; aktif iskeletsel büyümeden faydalanılan fonksiyonel ortodontik apareylerin zamanlamasında, implant uygulamaları öncesinde, ortognatik cerrahi ve dental travma vakalarının değerlendirilmesinde yaygın olarak kullanılmaktadır. İskeletsel olgunlaşmanın saptanmasında el-bilek radyografileri standart bir yöntem olarak kabul edilmekte ve bu alanda Greulich-Pyle, Tanner-Whitehouse, Björk ile Fishman yöntemleri öne çıkmaktadır. Ayrıca, dental panoramik radyografiler üzerinden Nolla ve Demirjian yöntemleriyle diş gelişim evreleri incelenmekte ve kemik yaşı ile uyumu değerlendirilmektedir. Son yıllarda ise lateral sefalometrik radyografiler üzerinden değerlendirilen servikal vertebral matürasyon yöntemi, ilave bir radyasyon dozu gerektirmemesi ve el-bilek grafileri kadar güvenilir olması nedeniyle önemli bir klinik avantaj sunmaktadır. Sonuç olarak ortodontide doğru tanı ve etkin tedavi için matürasyon aşamalarının doğru yorumlanması ve hastaya en uygun düşük radyasyonlu yöntemin seçilmesi gerekmektedir.

Growth-development (maturation) and bone age determination play a critical role in diagnosis, treatment planning, and treatment timing in orthodontics. Since chronological age does not always parallel individual biological development, it remains insufficient on its own, and parameters reflecting biological development, such as bone and dental age, are taken as a basis. Bone age is widely utilized in dentistry for timing functional orthodontic appliances that benefit from active skeletal growth, pre-implant planning, orthognathic surgery, and dental trauma assessments. Hand-wrist radiography is accepted as the standard method for determining skeletal maturation, with the Greulich-Pyle, Tanner-Whitehouse, Björk, and Fishman methods being prominent. Additionally, dental development stages are analyzed using the Nolla and Demirjian methods via panoramic radiographs to evaluate their compatibility with bone age. In recent years, the cervical vertebral maturation method, evaluated through lateral cephalometric radiographs, offers an important clinical advantage by eliminating the need for additional radiation and being as reliable as hand-wrist films. Consequently, accurate interpretation of maturation stages and selecting the most appropriate, low-radiation method are required for precise diagnosis and effective treatment in orthodontics.
