Kanal Yenilemede Kullanılan Solventlerin Değerlendirilmesi

Özet

Endodonti bilim dalında başarısız kök kanal tedavilerinin yenilenmesi (retreatment) rutin bir prosedür olup, bu işlem sırasında kanal içi dolgu maddeleri ve patların sökülmesi klinisyenler için oldukça zor ve zaman alıcı olabilmektedir. Bu süreci kolaylaştırmak amacıyla manuel, döner veya ultrasonik aletlerin yanı sıra çeşitli solventlerin (çözücülerin) kullanımı yaygındır. İncelenen araştırmalarda kloroform, halotan, terebentin, ksilen, okaliptol ve portakal yağı gibi geleneksel solventlerin yanı sıra Endosolv E, Endosolv R ve sitrik asit gibi güncel alternatiflerin güta-perka ve patlar üzerindeki çözücülük etkinlikleri ile biyo-uyumlulukları değerlendirilmiştir. Çalışmalar, kloroform ve ksilen gibi maddelerin yüksek çözücü etkiye sahip olduğunu gösterse de; bu kimyasalların kanserojen potansiyelleri, sitotoksik ve organik dokular üzerindeki toksik etkileri ciddi çekinceler yaratmaktadır. Daha güvenilir bir profil çizen portakal yağı ve okaliptol gibi doğal yağlar ile smear tabakasına da etki eden sitrik asit, kloroforma iyi birer alternatif olarak öne çıkmaktadır. Sonuç olarak, çözücü solventlerin klinisyenlere zaman açısından büyük kolaylık sağladığı kesin olmakla birlikte, kök kanallarını hiçbir metodun tamamen temizleyemediği ve bu maddelerin insan dokularına zarar verdiği kanıtlanmıştır. Bu nedenle, solventlerin mümkün mertebe kullanılmaması, endikasyon varlığında ise minimal düzeyde ve azami dikkatle tercih edilmesi önerilmektedir.


In the field of endodontics, nonsurgical retreatment of failed root canal therapies is a routine procedure, during which the removal of intra-canal filling materials and sealers can be highly challenging and time-consuming for clinicians. To facilitate this process, the use of various solvents is common alongside manual, rotary, or ultrasonic instruments. Reviewed studies evaluated the dissolving efficacies and biocompatibilities of traditional solvents such as chloroform, halothane, turpentine, xylene, eucalyptol, and orange oil, as well as current alternatives like Endosolv E, Endosolv R, and citric acid on gutta-percha and sealers. Although studies demonstrate that substances like chloroform and xylene possess high dissolving efficiency, their carcinogenic potential, cytotoxic effects, and toxicity on organic tissues raise serious concerns. Safer profiles such as orange oil and eucalyptol, alongside citric acid which also affects the smear layer, emerge as good alternatives to chloroform. Consequently, while solvents provide time-saving convenience for clinicians, it has been proven that no method completely cleans the canals and these substances cause toxic effects on human tissues. Therefore, it is recommended to avoid using these solutions if possible, and in cases with strict indications, to use them minimally and with ultimate care.
