Covid-19 Pandemisi Döneminde Yoğun Bakım Ünitesinde Trakeostomi Uygulamaları

Özet

COVID-19 pandemisi, yoğun bakım ünitelerinde uzun süreli mekanik ventilasyon ihtiyacını ve buna bağlı olarak trakeostomi uygulamalarını artırmıştır. Trakeostomi, aerosol yayılım riski taşıdığı için sağlık çalışanlarının güvenliği ve işlemin zamanlaması büyük önem arz etmektedir. Uluslararası kuruluşlar ve Sağlık Bakanlığı, viral yükün azalması amacıyla işlemin entübasyondan en erken 14 gün sonra yapılmasını önermektedir. Perkütan veya cerrahi yöntemler arasında kesin bir üstünlük bildirilmemekle birlikte, aerosol oluşumunu minimize edecek teknikler (tekli dilatasyon, koter kullanımından kaçınma vb.) tercih edilmelidir. İşlem sırasında N95/FFP2 maske, siperlik ve çift eldiven gibi tam kişisel koruyucu ekipman kullanımı zorunludur. Uygulama, en deneyimli hekim tarafından, mümkünse negatif basınçlı odalarda ve minimum personel ile gerçekleştirilmelidir.

The COVID-19 pandemic has increased the need for mechanical ventilation in intensive care units, leading to a rise in tracheostomy procedures. Since tracheostomy carries a risk of aerosol generation, healthcare worker safety and the timing of the procedure are critical. To reduce viral load, international organizations and the Ministry of Health recommend performing the procedure at least 14 days after intubation. While no clear superiority exists between percutaneous or surgical methods, techniques that minimize aerosol formation (e.g., single dilatation, avoiding cautery) should be preferred. The use of full personal protective equipment, including N95/FFP2 masks, face shields, and double gloves, is mandatory. The procedure should be performed by the most experienced clinician, preferably in negative pressure rooms with minimum staff.
