Özet

ERAS (Enhanced Recovery After Surgery - Cerrahi Sonrası İyileşmenin Hızlandırılması), cerrahi sonrası fonksiyonel iyileşmeyi hızlandırmayı ve metabolik yanıtları optimize etmeyi hedefleyen multidisipliner bir perioperatif bakım yaklaşımıdır. İlk olarak kolorektal cerrahi için geliştirilen bu protokoller; günümüzde toraks cerrahisi dahil pek çok alanda başarıyla uygulanmaktadır. Protokolün temel amacı, cerrahi stresi azaltarak hastanın fizyolojik fonksiyonlarına en kısa sürede dönmesini sağlamaktır. ERAS protokolü; preoperatif, intraoperatif ve postoperatif olmak üzere üç ana evreden oluşur. Preoperatif dönemde hastanın bilgilendirilmesi, nutrisyonel değerlendirme, sigara ve alkolün bıraktırılması ve ameliyat öncesi açlık süresinin berrak sıvılar için 2 saate indirilmesi gibi adımlar öne çıkar. İntraoperatif süreçte minimal invaziv cerrahi (VATS), kısa etkili anesteziklerin kullanımı, hipoterminin önlenmesi ve optimize edilmiş sıvı yönetimi esastır. Postoperatif dönemde ise erken oral alım, etkili ağrı yönetimi (özellikle epidural analjezi), erken mobilizasyon ve hava kaçaklarının önlenmesi kritik rol oynar. Yapılan çalışmalar, ERAS protokollerinin hastanede yatış süresini 2-3 gün, komplikasyonları ise %30-50 oranında azalttığını göstermektedir. Ayrıca uygulama oranı %70’i aştığında mortalitede %42 oranında düşüş gözlemlenmiştir. Sonuç olarak ERAS; maliyetleri düşüren, hasta yaşam kalitesini artıran ve iyileşme sürecini hızlandıran güvenilir bir sistemdir.

ERAS (Enhanced Recovery After Surgery) is a multidisciplinary perioperatice care approach aimed at accelerating functional recovery after surgery and optimizing metabolic responses. Initially developed for colorectal surgery, these protocols are now successfully implemented in many fields, including thoracic surgery. The primary goal of the protocol is to reduce surgical stress and ensure the patient returns to physiological functions as quickly as possible. The ERAS protocol consists of three main phases: preoperative, intraoperative, and postoperative. In the preoperative period, steps such as patient education, nutritional assessment, cessation of smoking and alcohol, and reducing preoperative fasting time to 2 hours for clear liquids stand out. During the intraoperative process, minimal invasive surgery (VATS), use of short-acting anesthetics, prevention of hypothermia, and optimized fluid management are essential. In the postoperative period, early oral intake, effective pain management (especially epidural analgesia), early mobilization, and prevention of air leaks play a critical role. Studies show that ERAS protocols reduce hospital stay by 2-3 days and complications by 30-50%. Furthermore, when the implementation rate exceeds 70%, a 42% reduction in mortality has been observed. In conclusion, ERAS is a reliable system that lowers costs, improves patient quality of life, and accelerates the healing process.
