Özet

Enürezis noktürna (EN), 5 yaş üzerindeki çocuklarda gece uykusunda istemsiz idrar kaçırma olarak tanımlanan, çocuk ve aile üzerinde sosyal ve duygusal yük oluşturan yaygın bir durumdur. Primer EN kesintisiz bir kuru dönemin olmamasını, sekonder EN ise en az 6 aylık kuru bir dönemden sonra başlayan idrar kaçırmayı ifade eder; bu bölümde ise sadece gece yatak ıslatma ile karakterize monosemptomatik EN üzerinde durulmuştur. Etiyolojide genetik faktörlerin yanı sıra yüksek uyanma eşiği, gece idrar miktarının artışı ve düşük mesane kapasitesi rol oynamaktadır. Tanı aşamasında detaylı anamnez, mesane günlüğü ve fizik muayene temel alınırken, ayırıcı tanı için idrar analizi ve gerekirse uyku problemleri sorgulanmalıdır. Tedavi yaklaşımında sıvı kısıtlaması gibi destekleyici yöntemler ve uyumlu ailelerde düşük relaps oranına sahip alarm tedavisi önceliklidir. Tıbbi tedavide ise noktürnal poliüri durumunda desmopressin %70 başarı sağlarken, dirençli vakalarda antikolinerjik kombinasyonları kullanılabilir; imipramin ise kardiyotoksisite riski nedeniyle ilk basamakta önerilmemektedir. Sonuç olarak, 5 yaş altı çocuklarda kendiliğinden düzelme sık görüldüğünden tedaviye daha ileri yaşlarda, hastanın ve ailenin durumu gözetilerek başlanmalıdır.

Nocturnal enuresis (NE) is a common condition defined as involuntary nighttime bedwetting in children over five years old, creating a social and emotional burden on both the child and the family. Primary NE refers to the absence of a continuous dry period, while secondary NE describes bedwetting after at least six months of dryness; this section focuses specifically on monosymptomatic NE characterized only by nighttime wetting. Hereditary factors, a high arousal threshold, increased nocturnal urine output, and low bladder capacity play key roles in its etiology. While diagnosis is primarily based on medical history, bladder diaries, and physical examination, urinalysis and screening for sleep disorders are utilized for differential diagnosis. Treatment approaches prioritize supportive methods like fluid restriction and alarm therapy, which has a high success rate and low relapse risk in compliant families. Regarding medical treatment, desmopressin achieves 70% success in cases of nocturnal polyuria, and anticholinergic combinations can be used for resistant cases, whereas imipramine is not recommended as a first-line treatment due to cardiotoxicity. Consequently, since spontaneous recovery is frequent in children under five, treatment should be initiated at later ages, considering the patient's and family's specific needs.
