Özet

Bu bilimsel çalışma, tiroid bezinin enflamasyonu olarak tanımlanan tiroiditlerin tanısında kullanılan radyolojik görüntüleme yöntemlerini ve bunlara ait bulguları kapsamlı bir şekilde incelemektedir. Tiroidit tanısının sadece radyolojik bulgulara dayanarak değil; fizik muayene, hasta geçmişi ve laboratuvar sonuçlarıyla birlikte yorumlanarak konulması gerektiği vurgulanmaktadır. En temel ve yaygın yöntem olan Renkli Doppler Ultrasonografi (RDUS) destekli tiroid ultrasonu (US); glandüler doku hacmi, parankim ekojenitesi ve vaskülarizasyon özelliklerini değerlendirmede kritik öneme sahiptir. Metinde; hipotiroidinin en sık nedeni olan ve "zürafa derisi" parankim paterniyle öne çıkan Hashimoto tiroiditi, şiddetli ağrıyla seyreden akut süpüratif ve subakut granülomatöz (De Quervain) tiroiditler, postpartum lenfositik tiroidit ile çevre dokulara invaziv özellik gösteren Riedel fibröz tiroiditinin karakteristik US/RDUS bulguları detaylandırılmıştır. Ayrıca, Graves hastalığında izlenen tipik "tiroid inferno" görünümü ile nadir görülen tüberküloz tiroiditinin bulgularına yer verilmiştir. Son yıllarda klinik kullanımı artan ve doku sertliğini kantitatif olarak ölçen Shear Wave Elastografi (SWE) teknolojisinin, tiroiditlerin evrelemesi ile parankimal fibrozis derecesini belirlemedeki kritik rolü belirtilmiştir. Bilgisayarlı Tomografi (BT), Manyetik Rezonans Görüntüleme (MRG) ve fonksiyonel veri sağlayan radyonüklid yöntemlerin ise ayırıcı tanıda tamamlayıcı roller üstlendiği ifade edilmektedir. Sonuç olarak ultrasonografik yaklaşımlar, doğru sınıflandırma ve uzun dönemli tedavi yönetimi için mükemmel bir rehberdir.

This scientific study comprehensively evaluates the radiological imaging modalities and their characteristic findings in the diagnosis of thyroiditis, which represents the inflammation of the thyroid gland. It emphasizes that a definitive diagnosis should not rely solely on radiology, but must integrate physical examination, clinical history, and laboratory results. Thyroid ultrasound (US) supported by Color Doppler Ultrasonography (CDUS) remains the fundamental tool, uniquely capable of assessing glandular volume, parenchymal echogenicity, and vascularity patterns. The text details specific ultrasonographic features of major conditions: Hashimoto's thyroiditis, characterized by its distinct "giraffe-hide" parenchymal pattern; painful acute suppurative and subacute granulomatous (De Quervain) thyroiditis; postpartum lymphocytic thyroiditis; and the highly invasive Riedel's fibrous thyroiditis. Furthermore, it highlights the striking "thyroid inferno" appearance characteristic of Graves' disease and discusses rare manifestations like tuberculous thyroiditis. The study underscores the emerging role of Shear Wave Elastography (SWE) as a quantitative tool to assess tissue stiffness, which directly correlates with the severity of inflammation and degree of fibrosis. Ultimately, while Computed Tomography (CT), Magnetic Resonance Imaging (MRI), and radionuclide scans offer crucial complementary data for differential diagnosis, US tracking remains indispensable for effective longitudinal patient management.
