Paratiroid Hastalıklarında Nükleer Tıp Yöntemleri

Özet

Paratiroid bezi endokrin sistemde kalsiyum metabolizması üzerinde önemli role sahip olup, paratiroid adenomları primer hiperparatiroidinin (PHPT) ana sorumlusudur. Cerrahi öncesinde hiperfonksiyonel bezlerin kesin lokalizasyonu, cerrahi başarının artırılması ve operasyon konforu açısından büyük öneme sahiptir. Geleneksel iki boyutlu planar paratiroid sintigrafisinden hibrit SPECT/BT ve PET/BT gibi üç boyutlu modalitelere geçiş, fonksiyonel ve morfolojik verileri birleştirerek anatomik lokalizasyonda büyük başarı sağlamıştır. Tc-99m MIBI, paratiroid sintigrafisinde en yaygın kullanılan radyofarmasötik olup çift izotop veya çift faz görüntüleme protokolleriyle uygulanabilmektedir. Çift fazlı görüntülemede SPECT/BT kullanımı, özellikle ektopik bezlerin saptanmasında planar ve standart SPECT yöntemlerine kıyasla üstün doğruluğa sahiptir. Son yıllarda öne çıkan F-18 florokolin ve C-11 metiyonin gibi PET/BT ajanları, yüksek uzaysal çözünürlükleri ve kısa çekim süreleriyle küçük lezyonların tespitinde yüksek sensitivite sergilemektedir. Birinci basamak yöntemlerin yetersiz veya negatif kaldığı karmaşık ya da tekrarlayan vakalarda; PET/BT, PET/MR ve 4D-BT gibi ileri ikinci basamak görüntüleme teknolojileri cerrahiye rehberlik etmede kritik avantajlar sunmaktadır.

The parathyroid gland plays a central role in regulating calcium metabolism, and parathyroid adenomas account for most primary hyperparathyroidism (PHPT) cases. Precise preoperative localization of hyperfunctioning glands is essential for guiding successful surgical interventions and achieving long-term cure. The technological shift from two-dimensional planar scintigraphy to three-dimensional hybrid modalities like SPECT/CT and PET/CT has revolutionized endocrine diagnosis by successfully combining functional and morphological data. Technetium-99m MIBI is the most widely utilized radiopharmaceutical in parathyroid scintigraphy, routinely applied via dual-isotope or dual-phase imaging protocols. Integrating SPECT/CT into dual-phase protocols offers superior diagnostic accuracy over planar methods, proving exceptionally beneficial for identifying ectopic glands. Recently, PET/CT imaging with positron-emitting tracers such as F-18 fluorocholine and C-11 methionine has demonstrated excellent sensitivity in detecting tiny lesions owing to higher spatial resolution and shorter acquisition times. In complex or recurrent cases where first-line modalities yield negative or inconclusive findings, advanced second-line techniques including PET/CT, PET/MRI, and 4D-CT offer vital diagnostic guidance for revision surgeries.
