Endotel Disfonksiyonunun Bir Belirteci; Endokan
Özet
Endotel, vasküler hemostazın korunmasında hayati bir rol oynayan, damarların iç yüzeyini kaplayan geniş bir metabolik organdır. Bu dokunun fonksiyonlarını yerine getirememesi sonucu gelişen endotel disfonksiyonu; başta ateroskleroz, hipertansiyon ve diyabet olmak üzere pek çok kardiyovasküler hastalığın temelini oluşturmaktadır. Endokan (ESM-1), 1996 yılında tanımlanan ve özellikle aktive olmuş vasküler endotelden salgılanan proteoglikan yapıda yeni bir biyobelirteçtir. Bu molekül; lökosit adezyonu, enflamasyon, anjiyogenez ve vasküler geçirgenlik gibi süreçlerde kritik bir modülatör olarak görev yapar. Klinik çalışmalar, serum endokan düzeylerinin sistemik enflamasyon ve endotel aktivasyonunun güçlü bir göstergesi olduğunu ortaya koymaktadır. Hipertansiyon hastalarında endokan seviyeleri damar sertliği ile pozitif korelasyon gösterirken, kronik böbrek yetmezliğinde hastalığın evresiyle birlikte artış sergilemektedir. Ayrıca sepsiste şiddet belirteci, kanserlerde ise tümör vaskülarizasyonu ve agresifliğiyle ilişkili bir prognostik faktör olarak dikkat çekmektedir. Henüz rutin klinik uygulamaya girmemiş olsa da endokan; vasküler patolojilerin erken teşhisi, risk sınıflandırması ve tedavi takibinde yüksek potansiyele sahip stratejik bir biyobelirteç adayıdır.
The endothelium is a vast metabolic organ lining the inner surface of vessels that plays a vital role in maintaining vascular homeostasis. Endothelial dysfunction, which occurs when this tissue fails to perform its functions, forms the basis of many cardiovascular diseases, primarily atherosclerosis, hypertension, and diabetes. Endocan (ESM-1), identified in 1996, is a novel proteoglycan-structured biomarker secreted specifically from the activated vascular endothelium. This molecule serves as a critical modulator in processes such as leukocyte adhesion, inflammation, angiogenesis, and vascular permeability. Clinical studies reveal that serum endocan levels are a potent indicator of systemic inflammation and endothelial activation. While endocan levels show a positive correlation with vascular stiffness in hypertensive patients, they exhibit an increase alongside the disease stage in chronic kidney failure. Furthermore, it stands out as a severity marker in sepsis and a prognostic factor associated with tumor vascularization and aggressiveness in cancers. Although not yet in routine clinical use, endocan is a strategic biomarker candidate with high potential for early diagnosis, risk stratification, and treatment monitoring of vascular pathologies.
