Kanser Hastalarında Sosyal Destek, Tedavi Reddi, Ötenazi ve İntihar Riski

Özet

Kanser hastalığı ve tedavi süreci bireylerde fiziksel, psikososyal ve ekonomik sorunlara yol açarak benlik algısını, tedavi uyumunu ve yaşam kalitesini olumsuz etkilemektedir. Bu zorlu süreçte maddi, duygusal, bilgisel ve yaygın alanlarda sağlanan olumlu sosyal destek; hastaların stresini azaltmakta, hastalığı kabulünü kolaylaştırmakta ve tedavi uyumu ile yaşam süresini artırmaktadır. Aile ve arkadaş çevresinin yanı sıra onkolog ve onkoloji hemşiresi gibi sağlık profesyonellerinden alınan destek de hastaların ruhsal dayanıklılığını güçlendirmekte ve hastalık deneyimini anlamlandırmalarına yardımcı olmaktadır. Buna karşın, sosyal desteğin yetersiz olması durumunda ileri evre kanser hastalarında tedavi reddi, aktif/pasif ötenazi talepleri, doktor yardımlı intihar arzusu ve intihar riski belirgin şekilde artış göstermektedir. Fiziksel güçsüzlük, ileri evre, ağrı ve semptom yükü gibi faktörler intihar riskini tetiklerken; güçlü bir hasta-onkolog iletişimi, kültürel/dini inançlar ve aile bağı koruyucu rol oynamaktadır. Sonuç olarak, kanser tedavisinin psikolog ve psikiyatristlerin de yer aldığı multidisipliner bir ekip tarafından yürütülmesi, hastaların yaşam kalitesini yükseltmek ve bu ekstrem eğilimleri önlemek adına kritik bir öneme sahiptir.

The cancer disease and its treatment process lead to physical, psychosocial, and economic problems in individuals, negatively affecting self-perception, treatment compliance, and quality of life. In this challenging process, positive social support provided in tangible, emotional, informational, and widespread areas reduces patients' stress, facilitates acceptance of the disease, and increases treatment compliance as well as survival time. In addition to family and friend circles, support received from healthcare professionals such as oncologists and oncology nurses strengthens patients' psychological resilience and helps them make sense of the illness experience. Conversely, in cases of inadequate social support, treatment refusal, active/passive euthanasia requests, physician-assisted suicide desires, and suicide risk significantly increase in advanced-stage cancer patients. While factors such as physical weakness, advanced stage, pain, and symptom burden trigger suicide risk; strong patient-oncologist communication, cultural/religious beliefs, and family cohesion play a protective role. Consequently, conducting cancer treatment through a multidisciplinary team that includes psychologists and psychiatrists is of critical importance to improve patients' quality of life and prevent these extreme tendencies.
