Asetabulum Kırıklarına Yaklaşım
Özet
Asetabulum kırıkları, pelvisin karmaşık 3 boyutlu anatomisi ve majör nörovasküler yapılara yakınlığı nedeniyle ortopedik travma cerrahisinin en zorlu alanlarından biridir. Gençlerde yüksek enerjili, yaşlılarda ise osteoporoza bağlı düşük enerjili travmalarla ortaya çıkan bu kırıklarda, posttravmatik osteoartriti önlemek için anatomik redüksiyon şarttır. Tanı ve sınıflandırmada direkt röntgenografiler (AP pelvis ve Judet) ile kırık hattının yönünü ve eklem içi fragmanları detaylandıran bilgisayarlı tomografi (BT) kritik rol oynar. Tedavi planlamasında cerrahlar arasında ortak dil sağlayan Judet-Letournel sistemi, kırıkları basit ve kompleks olarak ikiye ayırır. Deplasmanın 2 mm'den az olduğu ve eklem uyumunun korunduğu durumlarda konservatif tedavi uygulanırken; instabilite, eklem içi serbest fragman ve belirgin deplasman varlığında açık redüksiyon ve internal fiksasyon tercih edilir. Cerrahi başarı, kaliteli eklem redüksiyonuna bağlıdır ve fiksasyon genellikle plak ve vidalarla sağlanır. Kırığın lokalizasyonuna göre posterior yerleşimlerde Kocher-Langenbeck; anterior yerleşimlerde ise Modifiye Stoppa veya İlioinguinal gibi spesifik cerrahi yaklaşımlar seçilerek vital yapılar korunur ve başarılı sonuçlar elde edilir.
Acetabular fractures represent one of the most challenging areas in orthopedic trauma surgery due to the complex three-dimensional anatomy of the pelvis and its proximity to major neurovascular structures. Occurring via high-energy traumas in young individuals and low-energy traumas related to osteoporosis in the elderly, anatomical reduction is essential in these fractures to prevent long-term posttraumatic osteoarthritis. While direct radiographs (AP pelvis and Judet views) are utilized in diagnosis and classification, computed tomography (CT) plays a critical role in evaluating the three-dimensional structure, fracture line orientation, and intra-articular fragments. The Judet-Letournel system, which provides a common language among surgeons for treatment planning, divides fractures into simple and complex types. Conservative treatment involving radiographic follow-up is preferred when intra-articular displacement is less than 2 mm and joint congruence is maintained, whereas open reduction and internal fixation using reconstruction plates and screws are indicated in cases of instability, marginal impaction, or loose bodies. Surgical success depends heavily on high-quality anatomical joint reduction, and depending on the fracture pattern, specific surgical approaches such as the posterior Kocher-Langenbeck or anterior approaches like the Ilioinguinal and Modified Stoppa are selected to achieve optimal stabilization.
