Tanı Konulmamış Mitral Stenozlu Gebede Sezaryen Sonrası Ani Gelişen Sol Kalp Yetmezliğinin Yönetimi
Özet
Bilinen ek hastalığı olmayan 26 yaşındaki takipsiz ve primipar gebe, fetal distress ve intrauterin gelişme geriliği nedeniyle 36. haftada acil sezaryene alınmıştır. Preoperatif dönemde öksürük şikayeti nedeniyle COVID-19 şüphesiyle yaklaşılan hastaya, zor hava yolu ve aspirasyon riskini minimize etmek amacıyla "hızlı seri spinal anestezi" uygulanmıştır. Operasyon sorunsuz tamamlansa da, postoperatif 2. saatte taşikardi ve hipotansiyon, 4. saatte ise ciddi solunum yetmezliği gelişmiştir. Fizik muayenede bilateral ralleri saptanan ve entübe edilen hastanın ekokardiyografisinde, romatizmal kapak hastalığına bağlı ileri mitral stenoz (kapak alanı 0.8 cm²) ve akut sol kalp yetmezliği tespit edilmiştir. Yoğun bakımda mekanik ventilasyon, diüretik ve kardiyak destek tedavisi uygulanan hasta, postoperatif 48. saatte ekstübe edilmiş ve 5. günde servise nakledilmiştir. Bu olgu, özellikle takipsiz gebelerde doğum sonrası gelişen akut solunum yetmezliğinde, tanı konulmamış kalp kapak hastalıklarının mutlaka akılda tutulması gerektiğini vurgulamaktadır. Gebelikte artan intravasküler volüm ve kalp debisi, ciddi mitral stenozu olan hastalarda dekompansasyona ve hayatı tehdit eden pulmoner ödeme yol açabilmektedir. Bu tür riskli vakaların yönetimi; obstetrisyen, kardiyolog ve anestezistlerden oluşan multidisipliner bir yaklaşım gerektirmektedir.
A 26-year-old follow-up-free primiparous pregnant woman at 36 weeks' gestation was admitted for an emergency cesarean section due to fetal distress and intrauterine growth restriction. Due to a preoperative cough and suspected COVID-19, "rapid sequence spinal anesthesia" was performed to mitigate airway and aspiration risks. Although the surgery was completed without immediate complications, the patient developed tachycardia and hypotension at the 2nd postoperative hour, followed by severe respiratory failure at the 4th hour. Physical examination revealed bilateral rales, and following intubation, echocardiography identified advanced mitral stenosis (valve area 0.8 cm²) due to rheumatic heart disease and acute left heart failure. After receiving mechanical ventilation, diuretics, and cardiac support in the intensive care unit, the patient was extubated at 48 hours and transferred to the ward on the 5th day. This case emphasizes that undiagnosed valvular heart disease must be considered in follow-up-free pregnant women presenting with postpartum acute respiratory failure. The physiological increase in intravascular volume and cardiac output during pregnancy can lead to decompensation and life-threatening pulmonary edema in patients with severe mitral stenosis. Managing such high-risk cases requires a multidisciplinary approach involving obstetricians, cardiologists, and anesthesiologists.
