Genel Anestezi Sonrası Gelişen Negatif Basınç Akciğer Ödemi

Özet

Bilinen sistemik bir hastalığı bulunmayan, Amerikan Anestezistler Derneği (ASA) ASA IE, 23 yaşında erkek hasta genel anestezi altında apendektomi ameliyatına alındı. Hastanın ameliyat öncesi akciğer grafisi ve rutin tetkiklerinde patolojik bulguya rastlanmadı. Ameliyat sonrasında yeterli ekspiryum volümü oluşturan hasta ekstübe edildi. Hastada ekstübasyonu takiben ciddi ajitasyon ve inspiratuar efor gelişti. İnterkostal ve suprasternal çekilmeler, batın solunumu, toraksta rijidite gelişen hastanın periferik oksijen satürasyonu (SpO2) % 40’a kadar düştü. Yaklaşık beş dakika %100 oksijen desteği, pozitif basınçlı ventilasyon yapılan hastanın  oksijen (O2) satürasyonu 6 L dak-1 O2 desteğiyle %83’e kadar yükseltilebildi. Bu tedaviler sonrasında spontan solunumu düzelen, şuuru açılan, hafif dispne ve takipnesi olan hastanın bakılan kan gazında oksijen satürasyonu ve parsiyel oksijen basıncı düşük bulundu. Yoğun bakım ünitesi (YBÜ)’ne transfer edilen hastanın çekilen akciğer grafisi ve toraks bilgisayarlı tomografisinde (BT) bilateral yaygın infiltrasyonlar görüldü. Negatif basınç akciğer ödemi (NBAÖ) tanısı konulan hastanın tedavisinde diüretik, metilprednizolon, morfin ve bronkodilatörler kullanıldı. Hasta 24. saatin sonunda klinik, kan gazı ve radyolojik bulguları tamamen düzelmiş şekilde servise devredildi. Hastada gelişen bu klinik tablonun patofizyolojisi ve tedavisi literatür ışığında incelenecektir. 

A 23-year-old male patient with no known systemic disease, classified as American Society of Anesthesiologists (ASA) physical status I E, was taken into surgery for appendectomy under general anesthesia. No pathological findings were detected in the patient's preoperative chest X-ray and routine laboratory tests. Following the operation, the patient, who generated an adequate expiratory volume, was extubated. Subsequent to extubation, severe agitation and inspiratory effort developed in the patient. The peripheral oxygen saturation ($SpO_2$) of the patient, who developed intercostal and suprasternal retractions, abdominal breathing, and thoracic rigidity, dropped to as low as 40%. The oxygen ($O_2$) saturation of the patient, who received 100% oxygen support and positive pressure ventilation for approximately five minutes, could only be raised up to 83% with $6\text{ L min}^{-1}$ $O_2$ support. After these treatments, the patient's spontaneous breathing improved, consciousness returned to normal, and mild dyspnea and tachypnea were present; however, the arterial blood gas analysis revealed low oxygen saturation and partial pressure of oxygen. The patient was transferred to the intensive care unit (ICU), where bilateral diffuse infiltrates were observed on the chest X-ray and thoracic computed tomography (CT). In the treatment of the patient, who was diagnosed with negative pressure pulmonary edema (NPPE), diuretics, methylprednisolone, morphine, and bronchodilators were administered. At the end of the 24th hour, the patient was transferred to the ward with completely resolved clinical, blood gas, and radiological findings. The pathophysiology and treatment of this clinical picture that developed in the patient will be reviewed in the light of the literature.
