Özet

Çoğu intraoperatif aritmi geçici ve klinik olarak önemsiz olmakla birlikte, bazıları altta yatan patolojiyi (örn., miyokardiyal iskemi, elektrolit anormallikleri) gösterir. Bazıları ise prosedüre veya ilaca özgü etiyolojiye bağlıdır. Bazen aritmi intraoperatif hemodinamik bozukluğa neden olur. Aritmi gelişimine katkıda bulunması muhtemel faktörler preoperatif dönemde veya intraoperatif dönemde tanınarak yönetilebilir. Tiroidektomi için operasyona alınan bir hastada intraoperatif dirençli bradikardi ve hipotansiyon gelişmesi üzerine yapılan değerlendirmelerde akut miyokardiyal iskemi teşhisi belirlenmiştir. Bu makalede bu olgu ışığında intraoperatif bradikardi nedenleri ve bradikardiye yaklaşım prosedürlerinin tartışılması amaçlanmıştır.

"Although most intraoperative arrhythmias are transient and clinically insignificant, some indicate an underlying pathology (e.g., myocardial ischemia, electrolyte abnormalities). Others depend on procedure- or drug-specific etiologies. Occasionally, arrhythmia causes intraoperative hemodynamic instability. Factors likely to contribute to the development of arrhythmia can be recognized and managed during the preoperative or intraoperative period. In a patient undergoing a thyroidectomy, acute myocardial ischemia was diagnosed upon evaluations performed after the development of intraoperative refractory bradycardia and hypotension. In light of this case, this article aims to discuss the causes of intraoperative bradycardia and bradycardia management procedures.
