Penetran Yüz Travmalı Bir Pediyatrik Olguda Anestezi Yönetimi
Özet
Penetran yaralanmalar çoğu merkezde tüm pediyatrik travma başvurularının %10 ile %20'sini oluşturmaktadır (1). Penetran yaralanmaların tedavisi oldukça zor olabilir ve sıklıkla hızlı değerlendirme ve müdahale gerektirir. Kraniyofasiyal travma pediyatrik dönemde meydana gelen travmaların önemli bir bileşenidir (2, 3). Pediyatrik dönemde penetran kraniyofasiyal yaralanmaların en sık sebebi bıçak-silah veya kırık cam-metal parçalarıdır (4). Çocuklar, daha büyük baş kütle-vücut oranları nedeniyle kraniyofasiyal travmaya duyarlıdırlar (3, 5). Bununla birlikte, yüz kırıkları yetişkinlere göre daha az sıklıkta görülür. Kraniyofasiyal yaralanmalı çocuk hastalarda, çocuklar ve yetişkinler arasındaki anatomi ve fizyoloji farklılıkları göz önünde bulundurulmalı ve anestezi yönetimi buna göre yapılmalıdır. Bu yazıda, inşaat demiri ile kraniyofasiyal bölgede penetran yaralanma meydana gelen bir pediyatrik olgunun perioperatif anestezi yönetimi ele alınmıştır.
Penetrating injuries account for 10% to 20% of all pediatric trauma admissions in most centers (1). The management of penetrating injuries can be highly challenging, frequently requiring rapid evaluation and intervention. Craniofacial trauma is a significant component of traumas occurring during the pediatric period (2, 3). In the pediatric population, the most common causes of penetrating craniofacial injuries are knives/firearms or fragments of broken glass and metal (4). Children are particularly susceptible to craniofacial trauma due to their larger head-to-body mass ratio (3, 5). However, facial fractures occur less frequently compared to adults. In pediatric patients with craniofacial injuries, the anatomical and physiological differences between children and adults must be taken into consideration, and anesthesia management should be tailored accordingly. This article discusses the perioperative anesthesia management of a pediatric case who sustained a penetrating craniofacial injury caused by a construction rebar.
