Akciğer Hasarı Olan Multipl Travmalı Hastada Nöroaksiyel ve Çoklu Periferik Blok Uygulaması
Özet
Travma, morbidite ve mortalitenin en önemli nedenlerinden biri olup, özellikle çoklu yaralanmaları olan hastalarda anestezi yönetimi kritik bir öneme sahiptir. Bu vaka sunumunda, araç dışı trafik kazası sonucu sağ tibia, sağ ulna ve sol el falanks fraktürleri ile sol akciğer kontüzyonu ve karaciğer laserasyonu gelişen 20 yaşındaki erkek hastanın başarılı anestezi süreci ele alınmaktadır. Hastanın akciğer hasarı ve devam eden SARS-CoV-2 pandemisi nedeniyle havayolu manipülasyonu riskli bulunduğundan, genel anestezi yerine kombine rejyonal anestezi teknikleri tercih edilmiştir. Operasyon sürecinde sağ alt ekstremite için kombine spinal-epidural anestezi, sağ üst ekstremite için infraklaviküler blok ve sol üst ekstremite için aksiller blok eş zamanlı ve ardışık olarak uygulanmıştır. Ultrasonografi ve nörostimülatör rehberliğinde gerçekleştirilen bu işlemler, lokal anestezik dozlarının toksik limitleri aşmadan güvenle kullanılmasını sağlamıştır. Toplam 4 saat süren operasyon boyunca ek analjezik ihtiyacı duyulmamış ve herhangi bir komplikasyon gelişmemiştir. Postoperatif ağrı kontrolü epidural kateter ve intravenöz parasetamol ile başarıyla yönetilmiştir. Sonuç olarak, yüksek riskli travma hastalarında çoklu rejyonal anestezi kombinasyonlarının, komplikasyonları azaltmak ve iyileşme sürecini optimize etmek için etkili bir alternatif olduğu kanıtlanmıştır.
Trauma remains a leading cause of morbidity and mortality, making anesthesia management critical in patients with multiple injuries. This case report presents the successful anesthetic management of a 20-year-old male patient who sustained fractures of the right tibia, right ulna, and left hand phalanges, along with left lung contusion and liver laceration following an extra-vehicular traffic accident. Due to the patient's lung injury and the ongoing SARS-CoV-2 pandemic, airway manipulation was deemed risky; therefore, combined regional anesthesia techniques were preferred over general anesthesia. During the procedure, combined spinal-epidural anesthesia for the right lower extremity, an infraclavicular block for the right upper extremity, and an axillary block for the left upper extremity were performed simultaneously and sequentially. These procedures, guided by ultrasonography and neurostimulation, allowed for the safe use of local anesthetics without exceeding toxic limits. No additional analgesics were required during the four-hour operation, and no complications occurred. Postoperative pain was successfully managed via an epidural catheter and intravenous paracetamol. In conclusion, the combination of multiple regional anesthesia techniques proved to be an effective alternative for reducing complications and optimizing recovery in high-risk trauma patients.
