Ağır KOAH’lı Hastada Ultrason Eşliğinde İnterskalen Brakiyal Pleksus Bloğu İle Artroskopik Omuz Cerrahisi
Özet
62 yaşında bilinen kronik obstrüktif akciğer hastalığı (KOAH) olan kadın hasta 6 ay önce başlayan omuz ağrısı ve son 2 aydır giderek artan hareket kısıtlılığı şikayeti ile ortopedi polikliniğine başvurdu. Öykü, fizik muayene ve radyolojik incelemelerle sağ rotator manşet rüptürü saptanan hastaya artroskopik rotator manşet tamiri planlanıp preoperatif değerlendirme için anestezi konsültasyonu istendi. Preoperatif değerlendirmesinde yapılan tetkik ve konsültasyonlar sonucunda hastanın genel anestezi açısından pulmoner yüksek riskli olduğunun saptanması üzerine hastaya operasyon için rejyonel anestezi planlandı.
A 62-year-old female patient with a known history of chronic obstructive pulmonary disease (COPD) presented to the orthopedics outpatient clinic with complaints of shoulder pain that started 6 months ago and progressively increasing limitation of motion over the last 2 months. Following the evaluation of her history, physical examination, and radiological investigations, a right rotator cuff rupture was detected. An arthroscopic rotator cuff repair was planned, and an anesthesia consultation was requested for preoperative evaluation. As a result of the tests and consultations performed during the preoperative assessment, the patient was found to be at high pulmonary risk for general anesthesia, and regional anesthesia was planned for the operation.
