Kikuchi-Fujimoto Hastalığı: Multiorgan Yetmezliği İle Seyir

Özet

Kikuchi-Fujimoto hastalığı (KFH), genellikle genç erişkinlerde görülen, etiyolojisi tam olarak bilinmeyen ve çoğunlukla kendi kendini sınırlayan nadir bir nekrotizan lenfadenit tablosudur. Klinik olarak genellikle ateş ve servikal lenfadenopati ile seyretse de, sistemik lupus eritematozus (SLE) veya lenfoma gibi malignitelerle karışabilen karmaşık bir tablo sergileyebilir. Bu vaka sunumunda, literatürde oldukça nadir görülen, multiorgan yetmezliği ile seyreden ve fatal sonuçlanan 43 yaşında bir erkek hasta ele alınmaktadır. Hastada mediastinal lenfadenopati, ateş ve pansitopeni bulguları üzerine yapılan lenf nodu biyopsisi ile KFH tanısı kesinleştirilmiştir. Başlangıçta uygulanan kortikosteroid tedavisine olumlu yanıt alınsa da, taburculuk sonrası süreçte tablo hızla kötüleşmiştir. Hastada ilerleyici karaciğer nekrozu, akut böbrek hasarı, plevral efüzyon ve dissemine intravasküler koagülasyon (DİK) gelişmiştir. Yoğun bakım sürecinde uygulanan plazmaferez, hemodiyaliz ve geniş spektrumlu antibiyotik tedavilerine rağmen, çoklu organ yetmezliği ve derinleşen pansitopeni nedeniyle hasta kaybedilmiştir. Bu vaka, genellikle selim seyirli kabul edilen KFH’nin, nadir de olsa hayati organ tutulumları yaparak ölümcül bir seyir izleyebileceğini ve bu süreçte DİK gelişiminin kritik bir risk faktörü olduğunu vurgulamaktadır.

Kikuchi-Fujimoto disease (KFD) is a rare necrotizing lymphadenitis of unknown etiology, typically seen in young adults and characterized by a self-limiting course. While it clinically presents with fever and cervical lymphadenopathy, it can exhibit a complex picture that mimics systemic lupus erythematosus (SLE) or malignancies such as lymphoma. This case report presents a 43-year-old male patient with multi-organ failure and a fatal outcome, a progression that is exceedingly rare in the literature. The diagnosis of KFD was confirmed via lymph node biopsy following findings of mediastinal lymphadenopathy, fever, and pancitopenia. Although the patient initially responded well to corticosteroid therapy, the condition rapidly deteriorated after discharge. The patient developed progressive hepatic necrosis, acute renal injury, pleural effusion, and disseminated intravascular coagulation (DIC). Despite intensive care interventions, including plasmapheresis, hemodialysis, and broad-spectrum antibiotics, the patient succumbed to multi-organ failure and deepening pancitopenia. This case emphasizes that while KFD is generally considered benign, it can occasionally lead to fatal outcomes through vital organ involvement, and the development of DIC remains a critical risk factor during its progression.
