Diş Tedavisi Uygulanan Down Sendromlu Hastada Anestezi Yönetimi
Özet
Down sendromu (DS), fenotipik özelliklerinin yanı sıra konjenital kalp hastalıkları, mental-motor gerilik ve üst solunum yolu anomalileri gibi çoklu sistemik tutulumlarla seyreden en yaygın kromozomal bozukluktur. Bu hastalar, makroglossi, kısa boyun yapısı, obezite ve atlanto-oksipital instabilite gibi anatomik faktörler nedeniyle anestezi yönetimi açısından yüksek riskli ve zor entübasyon adayı olarak kabul edilirler. Ayrıca, hastane fobisi ve iletişim güçlükleri sebebiyle sergiledikleri agresif tutumlar, operasyon öncesi titiz bir psikolojik hazırlık ve sedasyon desteğini zorunlu kılmaktadır. Sunulan olguda, diş tedavisi planlanan 23 yaşında, obez ve atriyal septal defekt (ASD) öyküsü olan Down sendromlu bir erkek hastanın anestezi süreci ele alınmıştır. Preoperatif değerlendirmelerde Mallampati skoru 3 olarak belirlenen ve klinik ölçümleri zor entübasyona işaret eden hastaya, premedikasyon amacıyla midazolam uygulanmıştır. İndüksiyon sonrası klasik laringoskopi ile vokal kordların görüntülenememesi üzerine, kör entübasyon tekniğiyle Frova kateter kullanılarak başarılı bir hava yolu yönetimi sağlanmıştır. Operasyonun ardından sugammadeks ile kas gevşetici bloğu geri çevrilen hasta, sorunsuz bir şekilde ekstübe edilerek taburcu edilmiştir. Bu vaka, Down sendromlu hastalarda olası komplikasyonlara karşı tüm ekipmanların hazır bulundurulmasının ve multidisipliner bir yaklaşımın hayati önemini vurgulamaktadır.
Down syndrome (DS) is the most common chromosomal abnormality, characterized by phenotypic features and multisystemic involvements such as congenital heart diseases, mental-motor retardation, and upper airway anomalies. Due to anatomical factors including macroglossia, short neck, obesity, and atlanto-occipital instability, these patients are considered high-risk for anesthesia management and candidates for difficult intubation. Additionally, aggressive behaviors resulting from hospital phobia and communication difficulties necessitate meticulous psychological preparation and sedation support before the operation. The presented case examines the anesthesia process of a 23-year-old obese male patient with DS and a history of atrial septal defect (ASD) scheduled for dental treatment. In the preoperative evaluation, the patient, whose Mallampati score was 3 and clinical measurements indicated difficult intubation, was administered midazolam for premedication. After induction, when the vocal cords could not be visualized via classical laryngoscopy, successful airway management was achieved using a Frova catheter with a blind intubation technique. Following the operation, the neuromuscular block was reversed with sugammadex, and the patient was extubated without complications and discharged. This case emphasizes the vital importance of having all necessary equipment ready and maintaining a multidisciplinary approach against potential complications in patients with Down syndrome.
