Üroloji Servisinde Preoperatif Hasta Takibi
Özet
Ürolojik cerrahi geçirecek hastaların ameliyat öncesi preoperatif hazırlık ve takip süreçleri; fiziksel, psikolojik, yasal ve ameliyat sabahı hazırlığı olmak üzere dört temel aşamadan oluşmaktadır. Fiziksel hazırlık kapsamında tam kan, biyokimya, EKG ve idrar kültürü gibi rutin tetkikler yapılır; ASA sınıflaması ile kardiyak, pulmoner ve hepatobiliyer risk değerlendirmeleri gerçekleştirilir. Yaşlılar, gebeler, obez ve beslenme bozukluğu olan hastalar gibi özellikli gruplara özel yaklaşımlar sergilenirken, diyabet ve tiroid gibi komorbiditelerin yönetimi sağlanır. Profilaksi aşamasında, cerrahi alan enfeksiyonlarını önlemek adına doğru zamanlama ve dozda antibiyotik uygulaması, mekanik/farmakolojik yöntemlerle venöz tromboembolizm (VTE) profilaksisi ve antitrombotik tedavilerin düzenlenmesi kritik rol oynar. Psikolojik hazırlıkta hasta ve yakınlarının operasyon ve anestezi hakkında detaylı bilgilendirilmesi hedeflenirken; yasal hazırlıkta aydınlatılmış onam formunun iki nüsha olarak imzalatılması gerekmektedir. Son olarak ameliyat günü serviste; vital bulguların takibi, oral alımın kesilmesi, takı ve protezlerin çıkarılması gibi son kontroller tamamlanarak hasta güvenli bir şekilde ameliyathaneye teslim edilir.
The preoperative preparation and follow-up processes of patients undergoing urological surgery consist of four main stages: physical, psychological, legal, and preoperative morning preparation. Within the scope of physical preparation, routine tests such as complete blood count, biochemistry, ECG, and urine culture are performed; cardiac, pulmonary, and hepatobiliary risk assessments are evaluated using the ASA classification. While tailored approaches are implemented for special groups such as the elderly, pregnant women, obese patients, and those with nutritional deficiencies, comorbidities like diabetes and thyroid disorders are strictly managed. In the prophylaxis phase, administering antibiotics at the correct time and dosage to prevent surgical site infections, executing venous thromboembolism (VTE) prophylaxis through mechanical/pharmacological methods, and regulating antithrombotic therapies play a critical role. Psychological preparation aims to inform patients and their relatives thoroughly about the operation and anesthesia, whereas legal preparation requires obtaining signed informed consent forms in two copies. Finally, on the day of surgery in the ward, final checks such as monitoring vital signs, stopping oral intake, and removing jewelry or prostheses are completed, and the patient is safely transferred to the operating room.
