Özet

Dışkıda incelme, tıbbi literatürde 19. yüzyılın sonlarından itibaren rektal neoplazmlar ve kolorektal kanserlerin önemli bir belirtisi olarak tanımlanmış klinik bir bulgudur. Normal dışkılama süreci, diyet ve kolon motilitesiyle şekillenirken; Bristol Dışkı Skalası gibi görsel araçlar dışkı kıvamı ile barsak fonksiyonu arasındaki ilişkiyi anlamlandırmada yaygın olarak kullanılır. Dışkıda kalem veya şerit şeklinde incelme görülmesi, özellikle barsak lümenini daraltan patolojilere işaret eder. Bu durumun başlıca nedenleri arasında kolorektal polipler, kanserler, hemoroid, anal fissür ve inflamatuar barsak hastalıkları (Ülseratif kolit, Crohn) yer almaktadır. Ayrıca pelvik organ tümörleri veya prostat hiperplazisi gibi barsak dışı hastalıkların dışarıdan baskı yapması da bu semptoma yol açabilmektedir. İrritabl barsak sendromu gibi fonksiyonel bozukluklar da sıklıkla dışkıda incelme ile karakterize olsa da, bu semptomun birkaç günden uzun sürmesi durumunda altta yatan malignitelerin dışlanması kritiktir. Bu nedenle, kronikleşen dışkı incelmelerinde detaylı fizik muayene, laboratuvar tetkikleri ve özellikle kolonoskopik değerlendirme ile etiyolojik faktörlerin belirlenmesi hayati önem taşımaktadır.

Stool narrowing is a clinical finding defined in the medical literature since the late 19th century as a significant symptom of rectal neoplasms and colorectal cancers. While the normal defecation process is shaped by diet and colonic motility, visual tools such as the Bristol Stool Scale are widely used to interpret the relationship between stool consistency and bowel function. The appearance of pencil-like or ribbon-like stools specifically points to pathologies that narrow the intestinal lumen. The primary causes of this condition include colorectal polyps, cancers, hemorrhoids, anal fissures, and inflammatory bowel diseases such as Ulcerative colitis and Crohn's disease. Additionally, external pressure caused by extra-intestinal diseases like pelvic organ tumors or prostate hyperplasia can also lead to this symptom. Although functional disorders like irritable bowel syndrome are frequently characterized by stool narrowing, it is critical to exclude underlying malignancies if this symptom persists for more than a few days. Therefore, in cases of chronic stool narrowing, identifying the etiological factors through detailed physical examination, laboratory tests, and especially colonoscopic evaluation is of vital importance.
