Özet
Dışkıda kan saptanması, kolorektal kanser taraması veya diğer gastrointestinal şikayetlerin değerlendirilmesinde kritik bir bulgudur. Bu durum, gözle görülmeyen (gizli) veya aşikar kanama şeklinde ortaya çıkabilir. Dışkıda gizli kan tespiti için Fekal İmmünokimyasal Test (FİT) ve Guaiac tabanlı testler kullanılır; FİT, diyete bağlı kalmaması ve yalnızca insan hemoglobinine duyarlı olması nedeniyle daha yüksek hassasiyet sunar. Tarama çalışmaları, bu testlerin kolorektal kanser kaynaklı mortaliteyi önemli ölçüde azalttığını kanıtlamıştır. Testin pozitif çıkması durumunda, kanamanın nedenini belirlemek için kolonoskopi ve üst endoskopi temel tanı yöntemleridir. Standart endoskopilerin yetersiz kaldığı "şüpheli ince bağırsak kanaması" durumlarında ise kablosuz video kapsül endoskopisi, itme enteroskopisi veya derin ince bağırsak enteroskopisi gibi ileri tekniklere başvurulur. Kanamanın etiyolojisinde divertiküloz, anjiyodisplazi, kolit, polipler, kanser ve hemoroid gibi çok çeşitli faktörler yer almaktadır. Özellikle yaşlı hastalarda vasküler nedenler ve divertiküller ön plandayken, gençlerde inflamatuar bağırsak hastalıkları daha sık görülür. Sonuç olarak, dışkıda kan saptanan hastalarda multidisipliner bir yaklaşımla kanama odağının netleştirilmesi ve nedene yönelik etkin bir tedavi planının sunulması hayati önem taşımaktadır.
The detection of blood in the stool is a critical finding in colorectal cancer screening or the evaluation of other gastrointestinal complaints. This condition can present as occult (hidden) or overt bleeding. For occult blood detection, Fecal Immunochemical Test (FIT) and Guaiac-based tests are utilized; FIT offers higher sensitivity as it does not require dietary restrictions and is specifically sensitive to human hemoglobin. Screening studies have proven that these tests significantly reduce colorectal cancer-related mortality. If a test result is positive, colonoscopy and upper endoscopy are the primary diagnostic methods to determine the cause of bleeding. In cases of "suspected small bowel bleeding" where standard endoscopies are insufficient, advanced techniques such as wireless video capsule endoscopy, push enteroscopy, or deep small bowel enteroscopy are employed. The etiology of bleeding includes a wide range of factors such as diverticulosis, angiodysplasia, colitis, polyps, cancer, and hemorrhoids. While vascular causes and diverticula are more prominent in elderly patients, inflammatory bowel diseases are more common in younger individuals. Consequently, it is of vital importance to clarify the source of bleeding through a multidisciplinary approach and provide an effective, cause-oriented treatment plan for patients with blood detected in their stool.
