Anoskopi
Özet
Anoskopi, anüs ve distal rektumun iç yüzeyini doğrudan gözlemlemek için kullanılan, bağırsak hazırlığı veya sedasyon gerektirmeyen, hızlı, maliyet etkin ve pratik bir tanı yöntemidir. İnternal hemoroid, anal fissür, fistül, proktit ve anal kanser taraması gibi pek çok patolojinin teşhisinde altın standart kabul edilen bu işlem, dijital rektal muayene ile saptanamayan lezyonların belirlenmesinde fleksibl sigmoidoskopiye göre daha yüksek duyarlılığa sahiptir. Muayene sırasında anoskop adı verilen bir alet, topikal anestezi ve kayganlaştırıcı jel yardımıyla anüse yerleştirilir; uygun ışık kaynağı eşliğinde hekim ve hemşire iş birliğiyle bölge görselleştirilir. İşlem genellikle lateral dekübit pozisyonunda gerçekleştirilse de diz-omuz veya yüzüstü pozisyonlarda da uygulanabilir. Anoskopi, özellikle HIV pozitif hastalar gibi yüksek riskli gruplarda sitolojik örnekleme için kritik öneme sahipken ciddi kanama veya hastanın tolere edemediği durumlarda kontrendikedir. İşlem sonrası nadiren mukoza yırtılması veya enfeksiyon gibi komplikasyonlar görülse de genel olarak güvenli bir prosedürdür. Bununla birlikte anoskopi, proksimal gastrointestinal kanama odaklarını dışlayamadığı için anorektal kanamaların değerlendirilmesinde tek başına nihai bir yöntem olarak görülmemelidir.
Anoscopy is a rapid, cost-effective, and practical diagnostic method used to directly observe the internal surfaces of the anus and distal rectum, requiring no bowel preparation or sedation. Considered the gold standard for diagnosing various pathologies such as internal hemorrhoids, anal fissures, fistulas, proctitis, and anal cancer screening, this procedure has a higher sensitivity than flexible sigmoidoscopy in identifying lesions that cannot be detected by digital rectal examination. During the examination, an instrument called an anoscope is inserted into the anus using topical anesthesia and lubricating gel, and the area is visualized with the aid of a suitable light source through the collaboration of a physician and a nurse. Although the procedure is generally performed in the lateral decubitus position, it can also be carried out in the knee-chest or prone positions. While anoscopy is of critical importance for cytological sampling, especially in high-risk groups such as HIV-positive patients, it is contraindicated in cases of severe bleeding or when the patient cannot tolerate the procedure. Although complications such as mucosal tearing or infection are rarely seen after the procedure, it is generally a safe process. However, anoscopy should not be viewed as a definitive method on its own in the evaluation of anorectal bleeding, as it cannot exclude proximal gastrointestinal bleeding sources.
