Endoskopik Ultrasonografi (EUS)
Özet
Endoskopik ultrasonografi (EUS), endoskopi ve ultrasonografinin avantajlarını birleştiren, gastrointestinal sistem (GİS) ve çevre organların yüksek çözünürlüklü görüntülenmesini sağlayan ileri bir tıbbi teknolojidir. 1980'lerde geliştirilen ve Türkiye'de 1990 yılından itibaren uygulanmaya başlanan bu yöntem, radyal, lineer ve miniprob olmak üzere üç farklı cihaz tipiyle gerçekleştirilmektedir. Radyal EUS, 360 derecelik panoramik görüntü sunarak tanısal süreçlerde kullanılırken; lineer EUS, doku örneklemesine (ince iğne aspirasyonu) ve terapötik girişimlere imkan tanıyan geniş bir açıyla gerçek zamanlı izleme olanağı sağlar. EUS'un temel endikasyonları arasında GİS malignitelerinin evrelemesi, pankreatiko-biliyer kitlelerin tanımlanması ve mediastinal lezyonların değerlendirilmesi yer alır. Tanısal başarısının yanı sıra, sıvı koleksiyonlarının drenajı, çölyak pleksus blokajı ve çeşitli kistik lezyonların tedavisi gibi geniş bir terapötik yelpazeye sahiptir. İşlem sırasında perforasyon, kanama veya enfeksiyon gibi nadir komplikasyon riskleri bulunsa da, doğru teknik ve deneyimli uzmanlar eşliğinde bu riskler minimal düzeydedir. Sonuç olarak EUS, özellikle pankreatiko-biliyer hastalıkların yönetiminde altın standart kabul edilen, klinik uygulamaları her geçen gün genişleyen kritik bir minimal invaziv yöntemdir.
Endoscopic ultrasonography (EUS) is an advanced medical technology that combines the advantages of endoscopy and ultrasonography, providing high-resolution imaging of the gastrointestinal system (GIS) and surrounding organs. Developed in the 1980s and practiced in Turkey since 1990, this method is performed with three different types of devices: radial, linear, and miniprobe. While radial EUS provides a 360-degree panoramic view for diagnostic processes, linear EUS offers real-time monitoring with a wide angle that allows for tissue sampling (fine-needle aspiration) and therapeutic interventions. The primary indications for EUS include the staging of GIS malignancies, identification of pancreaticobiliary masses, and evaluation of mediastinal lesions. In addition to its diagnostic success, it has a wide therapeutic range such as drainage of fluid collections, celiac plexus blockade, and treatment of various cystic lesions. Although there are rare complication risks during the procedure such as perforation, hemorrhage, or infection, these risks are minimal when performed with the correct technique and by experienced specialists. Consequently, EUS is considered the gold standard in the management of pancreaticobiliary diseases, representing a critical minimally invasive method with clinical applications expanding every day.
