Sanal Kolonoskopi
Özet
Sanal kolonoskopi (BT kolonoskopi), kolorektal kanser ve polip taramasında kullanılan, minimal invaziv ve bilgisayarlı tomografi tabanlı bir görüntüleme yöntemidir. Konvansiyonel kolonoskopiye alternatif olarak geliştirilen bu teknik, 1 cm ve üzerindeki poliplerin saptanmasında %93 duyarlılık ve %97 seçicilik gibi yüksek bir başarı oranına sahiptir. Özellikle tam tamamlanamayan kolonoskopilerde, anestezi riskinin olduğu durumlarda veya hasta tercihine bağlı olarak tamamlayıcı veya birincil tarama testi olarak tercih edilmektedir. Yöntemin en büyük avantajları arasında sedasyon gerektirmemesi, perforasyon riskinin düşük olması ve kolon dışı yapıların da değerlendirilebilmesi yer alırken; radyasyon maruziyeti ve işlem sırasında biyopsi yapılamaması temel dezavantajlarıdır. Tetkik öncesinde barsak temizliği ve feçes işaretleme yöntemleri uygulanmakta, çekim sonrası 2 ve 3 boyutlu endolüminal görüntüler iş istasyonlarında uzmanlarca değerlendirilmektedir. Feçes, rektal kateterler ve yetersiz distansiyon gibi yanıltıcı durumlar tanıda zorluk çıkarabilse de gelişen teknoloji ile sanal kolonoskopinin gelecekte kolorektal kanser tanısında daha geniş yer bulması beklenmektedir.
Virtual colonoscopy (CT colonography) is a minimally invasive, computed tomography-based imaging method used for colorectal cancer and polyp screening. Developed as an alternative to conventional colonoscopy, this technique has a high success rate for detecting polyps 1 cm and larger, with a sensitivity of 93% and specificity of 97%. It is preferred as a complementary or primary screening test, particularly in cases of incomplete colonoscopy, when anesthesia risk exists, or based on patient preference. The primary advantages of the method include the lack of sedation requirement, low risk of perforation, and the ability to evaluate extra-colonic structures simultaneously; however, radiation exposure and the inability to perform biopsies during the procedure are its main disadvantages. Before the examination, bowel preparation and fecal tagging methods are applied, and after imaging, 2D and 3D endoluminal images are evaluated by specialists at workstations. Although pitfalls such as feces, rectal catheters, and insufficient distension can pose diagnostic challenges, virtual colonoscopy is expected to play a larger role in colorectal cancer diagnosis in the near future with advancing technology.
