Özet
Akut apandajit, epiploik apendiks dokusunun çoğunlukla iskemiye bağlı enflamasyonu olarak tanımlanan ve klinik pratikte nadir görülen bir tablodur. Anatomi açısından epiploik apendiksler, kolonun anti-mezenterik yüzeyinde bulunan, yağ dolu ve saplı peritoneal yapılardır. Genellikle epiploik apendiksin torsiyonu veya venöz tromboz sonucu kanlanmasının azalmasıyla etiyolojik olarak tetiklenir. En sık sigmoid kolon ve çekum bölgesinde yerleşen bu durum, sıklıkla ani başlangıçlı, noktasal ve iyi lokalize edilmiş karın ağrısı ile belirti verir. Laboratuvar bulguları genellikle normal sınırlarda olduğundan, tanı koymada bilgisayarlı tomografi (BT) kritik öneme sahiptir; BT'de izlenen "hyperattenuating ring sign" ve "central dot sign" karakteristik bulgulardır. Akut apandajit; divertikülit, akut apandisit ve omental infarkt gibi durumlarla karıştırılabildiğinden ayırıcı tanı büyük önem taşır. Tedavi yaklaşımı genellikle konservatif olup, hastaların büyük çoğunluğu anti-inflamatuar ilaçlarla ayaktan tedavi edilir; cerrahi müdahale veya antibiyotik kullanımı nadiren gerekir. Gereksiz cerrahi ve hastane maliyetlerini önlemek adına hekimlerin bu klinik tabloyu akılda tutması ve radyolojik görüntüleme bulgularına aşina olması gerekmektedir.
Acute appendagitis is a clinical condition characterized by the inflammation of the epiploic appendix tissue, usually due to ischemia, and is relatively rare in practice. Anatomically, epiploic appendages are fat-filled, pedunculated peritoneal structures located on the anti-mesenteric surface of the colon. It is etiologically triggered by the reduction of blood supply, often resulting from the torsion of the epiploic appendix or venous thrombosis. Most commonly situated in the sigmoid colon and cecum, this condition typically manifests as sudden-onset, pinpoint, and well-localized abdominal pain. Since laboratory findings are generally within normal limits, computed tomography (CT) is critical for diagnosis, where the "hyperattenuating ring sign" and "central dot sign" are characteristic findings. Differential diagnosis is vital as acute appendagitis can be confused with conditions such as diverticulitis, acute appendicitis, and omental infarction. The treatment approach is generally conservative, with the majority of patients treated as outpatients using anti-inflammatory medications; surgical intervention or antibiotics are rarely required. To prevent unnecessary surgeries and hospital costs, clinicians must keep this clinical entity in mind and be familiar with its diagnostic imaging findings.
