Özet
Bu makale, anorektal bölgeyi etkileyen cinsel yolla bulaşan hastalıkların (CYBH) fizyopatolojisi, korunma yöntemleri, tanı ve tedavi süreçlerini kapsamlı bir şekilde ele almaktadır. Giriş bölümünde anal bölgenin immünolojik ve fiziksel savunma mekanizmaları açıklanırken, özellikle anal ilişki yoluyla bulaşan proktit ve perianal ülserlerin klinik önemine vurgu yapılmaktadır. Herpes Simplex Virüsü (HSV), Human Papilloma Virüs (HPV), Gonore (Neisseria Gonorrhoeae), Klamidya (Chlamydia Trachomatis) ve Sifiliz (Treponema Pallidum) en sık görülen etkenler olarak sıralanmıştır. HSV'nin anorektal ağrı ve veziküler lezyonlara neden olduğu, HPV'nin ise kondiloma aküminata ve malignite riskiyle ilişkili olduğu belirtilmiştir. Gonore ve klamidya enfeksiyonlarının tanı ve ampirik tedavi yöntemleri detaylandırılırken, sifilizin evreleri ve karakteristik lezyonları incelenmiştir. Makalenin son bölümünde HIV pozitif bireylerde görülen anorektal komplikasyonlar, idiyopatik anal ülserler ve fırsatçı enfeksiyonlara (CMV ve MAC) odaklanılmıştır. Tanı süreçlerinde PCR, kültür ve serolojik testlerin kritik rolü vurgulanırken, tedavide antiviral ilaçlar, spesifik antibiyotik protokolleri ve cerrahi müdahalelerin gerekliliği üzerinde durulmuştur. Sonuç olarak, bu hastalıkların yönetiminde multidisipliner bir yaklaşım ve doğru tanı yöntemlerinin kullanımı büyük önem arz etmektedir.
This article comprehensively examines the pathophysiology, prevention methods, diagnosis, and treatment processes of sexually transmitted diseases (STDs) affecting the anorectal region. While the introduction explains the immunological and physical defense mechanisms of the anal area, the clinical importance of proctitis and perianal ulcers, especially those transmitted through anal intercourse, is emphasized. Herpes Simplex Virus (HSV), Human Papilloma Virus (HPV), Gonorrhea (Neisseria Gonorrhoeae), Chlamydia (Chlamydia Trachomatis), and Syphilis (Treponema Pallidum) are listed as the most common agents. It is noted that HSV causes anorectal pain and vesicular lesions, while HPV is associated with condyloma acuminata and malignancy risk. Diagnostic and empirical treatment methods for gonorrhea and chlamydia infections are detailed, and the stages and characteristic lesions of syphilis are examined. The final part of the article focuses on anorectal complications, idiopathic anal ulcers, and opportunistic infections (CMV and MAC) seen in HIV-positive individuals. While the critical role of PCR, culture, and serological tests in diagnostic processes is highlighted, the necessity of antiviral drugs, specific antibiotic protocols, and surgical interventions in treatment is emphasized. In conclusion, a multidisciplinary approach and the use of accurate diagnostic methods are of great importance in the management of these diseases.
