Özet

Pankreas, morfolojik ve işlevsel olarak ekzokrin ve endokrin olmak üzere iki ana bölüme ayrılan, sindirim ve metabolizma süreçlerinde hayati rol oynayan bir organdır. Bezin yaklaşık %90'ını oluşturan ekzokrin kısım, "asinus" adı verilen fonksiyonel birimlerden meydana gelir ve duodenuma boşaltılan sindirim enzimlerinin sentezinden sorumludur. Seröz salgı hücrelerinden oluşan bu yapılar, zimojen granülleri aracılığıyla tripsin, amilaz ve lipaz gibi enzimleri inaktif veya aktif formda salgılar. Salgı süreci, duodenumdan salınan sekretin ve kolesistokinin hormonları ile vagus siniri tarafından titizlikle kontrol edilir. Endokrin bölüm ise Langerhans adacıklarından oluşur ve kan şekeri düzenlenmesinde kritik öneme sahip olan insülin (beta hücreleri), glukagon (alfa hücreleri), somatostatin (delta hücreleri) ve pankreatik polipeptid (F hücreleri) gibi hormonları doğrudan kana verir. İnsülo-asinar portal sistem sayesinde endokrin hormonlar, ekzokrin doku üzerinde yerel bir etki göstererek organ içi koordinasyonu sağlar. Özellikle B hücrelerinde preproinsülinden üretilen insülin, glukoz metabolizmasını yönetirken; alfa hücrelerinden salınan glukagon, kan şekerini artırıcı etki yapar. Bu karmaşık histolojik yapı, hem besinlerin sindirilmesini hem de vücudun enerji dengesinin korunmasını sağlayarak homeostasisin devamlılığına katkıda bulunur.

The pancreas is an organ divided into two main sections, exocrine and endocrine, both morphologically and functionally, playing a vital role in digestive and metabolic processes. The exocrine part, which constitutes approximately 90% of the gland, consists of functional units called acini and is responsible for synthesizing digestive enzymes discharged into the duodenum. These structures, composed of serous secretory cells, secrete enzymes such as trypsin, amylase, and lipase in inactive or active forms via zymogen granules. The secretion process is strictly controlled by the hormones secretin and cholecystokinin released from the duodenum, as well as the vagus nerve. The endocrine section consists of islets of Langerhans and releases hormones critical for blood sugar regulation, such as insulin (beta cells), glucagon (alpha cells), somatostatin (delta cells), and pancreatic polypeptide (F cells), directly into the bloodstream. Thanks to the insulo-acinar portal system, endocrine hormones exert a local effect on the exocrine tissue, ensuring intra-organ coordination. In particular, while insulin produced from preproinsulin in B cells manages glucose metabolism, glucagon released from alpha cells has a blood sugar-increasing effect. This complex histological structure contributes to the maintenance of homeostasis by ensuring both the digestion of nutrients and the preservation of the body's energy balance.
