Pankreas Yetmezliği

Özet

Pankreas yetmezliği, özellikle sindirim için gerekli olan enzimlerin lümen içerisindeki aktivitesinin azalmasıyla karakterize, yaşam kalitesini ciddi oranda düşüren klinik bir durumdur. Yetişkinlerde en yaygın etiyolojik neden kronik pankreatit iken, çocuklarda kistik fibroz ön plana çıkmaktadır. Hastalık; steatore, karın ağrısı, kilo kaybı ve malnütrisyon gibi semptomlarla kendini gösterir ve tedavi edilmediğinde osteoporoz veya kardiyovasküler olaylar gibi ciddi komplikasyonlara yol açabilir. Tanı sürecinde fekal elastaz-1 testi en yaygın kullanılan yöntemdir; ancak 72 saatlik fekal yağ ölçümü (CFA) altın standart kabul edilmektedir. Tedavinin temelini, sindirim sürecini normale döndürmeyi amaçlayan pankreatik enzim replasman tedavisi (PERT) ve yaşam tarzı değişiklikleri oluşturur. Ayrıca pankreatik psödokistler ve kronik ağrı yönetimi, hastalığın prognozu açısından kritik öneme sahiptir. Erken teşhis ve uygun enzim desteği, morbidite ve mortalite riskini azaltarak hastaların yaşam süresini ve kalitesini bağımsız bir şekilde iyileştirmektedir.

Pancreatic insufficiency is a clinical condition characterized by the decreased activity of enzymes necessary for digestion within the intestinal lumen, significantly impairing the quality of life. While chronic pancreatitis is the most common etiological cause in adults, cystic fibrosis is more prevalent in children. The disease manifests with symptoms such as steatorrhea, abdominal pain, weight loss, and malnutrition, and if left untreated, it can lead to serious complications such as osteoporosis or cardiovascular events. In the diagnostic process, the fecal elastase-1 test is the most widely used method, although the 72-hour fecal fat measurement (CFA) is considered the gold standard. The cornerstone of treatment is pancreatic enzyme replacement therapy (PERT), which aims to normalize the digestive process, alongside lifestyle modifications. Furthermore, the management of pancreatic pseudocysts and chronic pain is critical for the prognosis of the disease. Early diagnosis and appropriate enzyme supplementation independently improve the lifespan and quality of life by reducing the risk of morbidity and mortality.
