Pankreas Kitlelerinde Radyolojinin Yeri
Özet
Pankreas kitlelerinin teşhis ve yönetiminde radyolojinin kritik rolünü ele alan bu bilimsel metin, modern görüntüleme teknolojilerinin anatomik detayları belirlemedeki gücünü vurgulamaktadır. Pankreas; baş, boyun, gövde ve kuyruk bölümlerinden oluşan, hem endokrin hem de ekzokrin fonksiyonlara sahip bir organ olup, anatomisinin net bilinmesi patolojik durumların tespiti için temel teşkil etmektedir. Görüntüleme yöntemleri arasında ultrason (US), maliyeti düşük ve radyasyonsuz bir ilk seçenek iken; bilgisayarlı tomografi (BT) yüksek uzaysal çözünürlüğü sayesinde özellikle adenokanserlerin evrelemesinde ve damar tutulumunun belirlenmesinde altın standart kabul edilir. Manyetik rezonans görüntüleme (MRG) ve MRCP ise kistik yapıların değerlendirilmesinde ve radyasyon riskinden kaçınılan durumlarda üstünlük sağlar. Metinde, pankreasın en yaygın malignitesi olan duktal adenokanserlerin yanı sıra nöroendokrin tümörler, solid psödopapiller neoplaziler ve pankreatoblastom gibi nadir görülen kitlelerin radyolojik karakteristikleri ayrıntılarıyla açıklanmıştır. Özellikle kistik lezyonların ayrımı için geliştirilen morfolojik sınıflama, cerrahi gerektiren malign potansiyelli vakaların saptanmasında yol göstericidir. Sonuç olarak radyoloji; biyobelirteçler ve klinik bulgularla birleştiğinde, gereksiz girişimsel işlemlerin önlenmesi ve doğru tedavi stratejilerinin planlanması açısından vazgeçilmez bir rehberdir.
This scientific text, which discusses the critical role of radiology in the diagnosis and management of pancreatic masses, emphasizes the power of modern imaging technologies in determining anatomical details. The pancreas is an organ consisting of the head, neck, body, and tail sections with both endocrine and exocrine functions, and a clear understanding of its anatomy is fundamental for detecting pathological conditions. Among imaging methods, ultrasonography (US) is a low-cost and radiation-free first option, while computed tomography (CT) is considered the gold standard for staging adenocarcinomas and determining vascular involvement due to its high spatial resolution. Magnetic resonance imaging (MRI) and MRCP provide superiority in evaluating cystic structures and in cases where radiation risk is avoided. The text details the radiological characteristics of ductal adenocarcinomas, the most common malignancy of the pancreas, as well as rarer masses such as neuroendocrine tumors, solid pseudopapillary neoplasms, and pancreatoblastoma. Particularly, the morphological classification developed for the differentiation of cystic lesions is guiding in detecting cases with malignant potential that require surgery. Consequently, when combined with biomarkers and clinical findings, radiology serves as an indispensable guide for preventing unnecessary invasive procedures and planning accurate treatment strategies.
