Özet

Bu yazı, pankreas gövde ve kuyruk tümörlerinde uygulanan cerrahi yöntemleri, özellikle distal pankreatektomi prosedürlerini kapsamlı bir şekilde ele almaktadır. Pankreasın anatomik yapısı ve damarsal beslenmesiyle başlayan metin, distal pankreas tümörlerinin genellikle geç evrede teşhis edildiğini ve düşük rezektabilite oranlarına sahip olduğunu vurgulamaktadır. Cerrahi teknikler arasında klasik distal pankreatektominin yanı sıra, çölyak aks tutulumu olan vakalarda uygulanan "Modifiye Appleby Operasyonu" (DP-CAR) detaylandırılmıştır. Bu yöntemin R0 rezeksiyon şansını artırdığı ve yaşam kalitesini iyileştirdiği belirtilmektedir. Ayrıca, dalak koruyucu yaklaşımlar olan Warshaw ve Kimura prosedürleri karşılaştırılmış; Kimura yönteminin teknik zorluğuna rağmen dalak fonksiyonlarını korumadaki avantajlarına değinilmiştir. Güncel teknolojik gelişmeler ışığında laparoskopik ve robotik cerrahinin açık cerrahiye göre daha az komplikasyon ve daha hızlı iyileşme gibi faydalar sağladığı, ancak ileri evre vakalarda açık cerrahinin halen standart olduğu ifade edilmiştir. Son olarak, ameliyat sonrası beslenme, antibiyotik ve enzim tedavisi gibi klinik yönetim stratejileri ile cerrahi endikasyonlar özetlenmiştir.

This article comprehensively covers surgical methods for pancreatic body and tail tumors, specifically focusing on distal pancreatectomy procedures. Starting with the anatomical structure and vascular supply of the pancreas, the text emphasizes that distal pancreatic tumors are often diagnosed at late stages and have low resectability rates. Among surgical techniques, besides classical distal pancreatectomy, the "Modified Appleby Operation" (DP-CAR), applied in cases with celiac axis involvement, is detailed. It is stated that this method increases the chance of R0 resection and improves quality of life. Furthermore, spleen-preserving approaches such as the Warshaw and Kimura procedures are compared, highlighting the advantages of the Kimura method in preserving splenic functions despite its technical difficulty. In light of current technological developments, it is noted that laparoscopic and robotic surgeries provide benefits like fewer complications and faster recovery compared to open surgery; however, open surgery remains the standard for advanced cases. Finally, clinical management strategies such as postoperative nutrition, antibiotic, and enzymatic therapy, along with surgical indications, are summarized.
