Pankreas Cerrahisinin Egzokrin Komplikasyonları ve Tedavi Yöntemleri

Özet

Pankreas cerrahisi sonrası gelişen pankreas egzokrin yetmezliği (PEY), hastaların yaşam kalitesini, beslenmesini ve sağ kalım oranlarını doğrudan etkileyen yaygın bir komplikasyondur. Literatürde PEY görülme oranları cerrahi tekniğe ve tanı yöntemlerine bağlı olarak %33 ile %100 arasında değişmekle birlikte, belirtilerin spesifik olmaması nedeniyle tanı genellikle gecikmektedir. Pankreatik enzimler sindirimde hayati rol oynar; eksikliği durumunda steatore (yağlı dışkı), şiddetli malnütrisyon ve özellikle A, D, E, K gibi yağda çözünen vitaminlerin emilim bozukluğu görülür. Tanıda fekal elastaz-1 testi yaygın olarak kullanılsa da rezeksiyon sonrası etkinliği tartışmalıdır; bu nedenle hastaların beslenme düzeyleri ve biyokimyasal markerları yakından izlenmelidir. Pankreatik enzim replasman tedavisi (PERT), PEY yönetiminde oldukça etkili ve güvenli bir yöntemdir. Tedavi süreci, ana öğünlerde ve atıştırmalıklarda uygun enzim dozlarının alınmasını, vitamin-mineral desteğini ve mide asidini dengelemek için proton pompa inhibitörü kullanımını içermektedir. Özellikle malignite cerrahisi sonrası rutin replasman önerilmekte ve bu tedavinin sağ kalımı artırdığı vurgulanmaktadır. Sonuç olarak, her pankreas cerrahisi hastasında PEY riski akılda tutulmalı ve erken dönemde müdahale edilmelidir.

Pancreatic exocrine insufficiency (PEI) is a common complication following pancreatic surgery that significantly affects patients' quality of life, nutrition, and survival rates. Although PEI incidence in the literature ranges from 33% to 100% depending on the surgical technique and diagnostic methods, diagnosis is often delayed due to non-specific symptoms. Pancreatic enzymes play a vital role in digestion; their deficiency leads to steatorrhea, severe malnutrition, and malabsorption of fat-soluble vitamins, particularly A, D, E, and K. While the fecal elastase-1 test is widely used in diagnosis, its efficacy after resection remains controversial; therefore, patients' nutritional levels and biochemical markers must be closely monitored. Pancreatic enzyme replacement therapy (PERT) is a highly effective and safe method for managing PEI. The treatment process involves taking appropriate enzyme doses during main meals and snacks, vitamin-mineral supplementation, and the use of proton pump inhibitors to balance gastric acid. Routine replacement is especially recommended after malignancy surgery, emphasizing that this treatment increases survival. Consequently, the risk of PEI should be kept in mind for every pancreatic surgery patient, and early intervention should be initiated.
