Özet

Afazi, beynin dil üretimi ve işlemleme bölgelerindeki hasarlar nedeniyle oluşan ve iletişim kısıtlılıklarına yol açan edinilmiş nörolojik bir bozukluktur. Bu durum sadece dil işlevlerini etkilemekle kalmayıp, hastalar ve bakım verenler üzerinde fiziksel, sosyal ve emosyonel düzeyde derin olumsuz etkiler yaratmaktadır. Bu çalışmada, afaziye eşlik eden komorbid nöropsikiyatrik bozukluklar kapsamlı şekilde ele alınmaktadır. En sık karşılaşılan nöropsikiyatrik durum inme sonrası depresyon olup, bunu anksiyete, apati, mani ve düşük oranda psikotik bozukluklar izlemektedir. DSM-5 ölçütlerine göre bu durumlar "Diğer Tıbbi Durumlara Bağlı Bozukluklar" kapsamında değerlendirilmektedir. Nöropsikiyatrik semptomların sıklığı ve şiddeti hastanın yaşı, cinsiyeti, eğitim düzeyi ve lezyon konumu gibi biyopsikososyal faktörlere bağlı olarak değişkenlik gösterir. Afazili bireylerin sosyal ortamlardan uzaklaşması ve motivasyon kaybı yaşaması günlük iletişimi zorlaştırmaktadır. Bu süreçte başarı sağlamak için hem hastanın hem de hasta yakınlarının farkındalık, psikolojik dayanıklılık ve empati becerilerinin geliştirilmesi kritik öneme sahiptir. Sonuç olarak, afazi rehabilitasyonunda hastaların etkin iletişim döngüsünde kalabilmesi amacıyla dil ve konuşma terapisinin yanı sıra psikoterapi ve ergoterapiyi de içeren bütüncül, multidisipliner bir yaklaşım benimsenmeli ve hasta yakınları da bu sürece aktif olarak dahil edilmelidir.

Aphasia is an acquired neurological disorder caused by damage to the brain regions responsible for language production and processing, leading to significant communication limitations. This condition not only impairs language functions but also exerts profound negative effects on patients and caregivers at physical, social, and emotional levels. This study extensively examines the comorbid neuropsychiatric disorders accompanying aphasia. The most frequently observed neuropsychiatric condition is post-stroke depression, followed by anxiety, apathy, mania, and low rates of psychotic disorders. According to DSM-5 criteria, these conditions are classified under "Disorders Due to Another Medical Condition". The frequency and severity of neuropsychiatric symptoms vary based on biopsychosocial factors such as the patient's age, gender, educational background, and lesion location. The tendency of individuals with aphasia to withdraw from social environments and experience a loss of motivation further complicates daily communication. In this process, fostering awareness, psychological resilience, and empathy skills in both patients and their relatives is of critical importance. In conclusion, to ensure that patients remain within the active daily communication cycle during aphasia rehabilitation, a holistic and multidisciplinary approach including psychotherapy and occupational therapy alongside speech and language therapy must be adopted, and caregivers must be actively supported within this program.
