Vardiyalı Sistemde Çalışmanın Bedensel, Ruhsal ve Sosyal Etkileri

Özet

Vardiyalı çalışma sistemi, biyolojik ritimleri bozarak bireylerin bedensel, ruhsal ve sosyal sağlığı üzerinde ciddi olumsuz etkilere yol açmaktadır. Suprakiazmatik nükleus tarafından yönetilen sirkadiyen ritim, aydınlık-karanlık döngüsü gibi çevresel zamanlayıcılarla senkronize olur; ancak gece çalışması bu uyumu bozarak melatonin, kortizol ve vücut sıcaklığı ritimlerinde çatışmaya neden olur. Biyolojik saatle gelişen bu uyumsuzluk, homeostatik uyku basıncı ile sirkadiyen süreç arasında çatışma yaratarak uyku süresini kısaltır, uykuyu böler ve vardiyalı çalışma tipi uyku bozukluğu ile insomniayı tetikler. Bu kronik ritim bozuklukları; hormonal düzensizlikler (TSH ve prolaktin değişimleri), metabolik sendrom, obezite, gastrointestinal şikayetler, diyabet ve bağışıklık sisteminin baskılanmasıyla artan kanser riskini (meme, prostat) beraberinde getirir. Kardiyovasküler sistemde ise miyokart enfarktüsü ve iskemik kalp hastalığı riskini yükseltir. Ruhsal alanda anksiyete, depresyon, somatizasyon, intihar düşüncesi ve madde kullanım eğilimi artarken; yorgunluğa bağlı bilişsel işlev bozuklukları, bellek zayıflığı ve iş kazası riskleri (gece vardiyasında 6-14 kat) katlanır. Ayrıca, toplumsal ritmin dışına çıkılması aile içi çatışmalara, ebeveynlik rollerinde aksamalara, sosyal izolasyona ve sağlık çalışanlarında tükenmişlik sendromuna yol açar. Sorunların şiddeti yaş, cinsiyet ve kronotip (sabah/akşam tipi) gibi bireysel farklılıklara göre değişmektedir.

The shift work system disrupts biological rhythms, leading to severe negative impacts on individuals' physical, mental, and social health. The circadian rhythm, managed by the suprachiasmatic nucleus, synchronizes with environmental zeitgebers like the light-dark cycle; however, night work disrupts this alignment, causing a conflict in melatonin, cortisol, and body temperature rhythms. This mismatch with the biological clock creates a conflict between homeostatic sleep pressure and the circadian process, shortening sleep duration, fragmenting sleep, and triggering shift work sleep disorder and insomnia. These chronic rhythm disruptions bring along hormonal irregularities (changes in TSH and prolactin), metabolic syndrome, obesity, gastrointestinal complaints, diabetes, and an increased risk of cancer (breast, prostate) due to immune suppression. In the cardiovascular system, it elevates the risk of myocardial infarction and ischemic heart disease. Mentally, anxiety, depression, somatization, suicidal ideation, and substance use tendencies increase, while fatigue-related cognitive dysfunction, memory impairment, and occupational accident risks (6-14 times higher in night shifts) multiply. Additionally, operating outside the social rhythm leads to familial conflicts, disruptions in parental roles, social isolation, and burnout syndrome in healthcare workers. The severity of these problems varies based on individual differences such as age, gender, and chronotype (morning/evening type).
