Özet

Kronobiyoloji, canlılardaki sirkadiyen ritimlerin ve biyolojik süreçlerin 24 saatlik döngüdeki işleyişini inceleyen bir bilim dalı olup, bu mekanizmaların bozulması ruhsal hastalıkların patofizyolojisinde kritik bir rol oynamaktadır. Araştırmalar; majör depresif bozukluk, bipolar bozukluk ve şizofreni gibi majör psikiyatrik rahatsızlıklarda uyku-uyanıklık döngüsü, melatonerjik sistem, hipotalamus-hipofiz-adrenal (HPA) ekseni ve inflamatuar süreçlerin sirkadiyen ritim anormallikleri ile yakından ilişkili olduğunu göstermektedir. Depresyonda uyku mimarisinin değişmesi ve HPA ekseni disregülasyonu öne çıkarken; bipolar bozuklukta duygudurum ataklarının mevsimselliği, kronotipler, genetik polimorfizmler ve sosyal sirkadiyen ritim bozulmaları (jet lag gibi) patogeneze katkıda bulunmaktadır. Şizofrenide ise striatal hiperdopaminerjik aktivite ile melatonin salınımındaki faz kaymalarının sirkadiyen saat genlerindeki (Clock, Period, Timeless) değişimlerle bağlandığı görülmektedir. Ayrıca, anksiyete, obsesif-kompulsif bozukluk ve madde kullanım bozukluklarında da sirkadiyen biyolojinin işlevsel kayıplara yol açtığı saptanmıştır. Sonuç olarak, sirkadiyen sistemin yeniden senkronizasyonunu hedefleyen ışık ve uyandırma gibi kronoterapötik yaklaşımlar, psikiyatrik bozuklukların tedavisinde ve nükslerin önlenmesinde güçlü, güvenli ve yenilikçi bir potansiyel sunmaktadır.

Circadian rhythms and biological processes regulated by chronobiology maintain a 24-hour cycle essential for health, and their disruption plays a critical role in the pathophysiology of mental illnesses. Research demonstrates that circadian rhythm abnormalities in major psychiatric disorders, such as major depressive disorder, bipolar disorder, and schizophrenia, are closely linked to alterations in sleep-wake cycles, the melatonergic system, the hypothalamic-pituitary-adrenal (HPA) axis, and inflammatory processes. While altered sleep architecture and HPA axis dysregulation are prominent in depression, the seasonality of mood episodes, chronotypes, genetic polymorphisms, and social circadian disruptions (such as jet lag) contribute to pathogenesis in bipolar disorder. In schizophrenia, striatal hyperdopaminergic activity and phase shifts in melatonin secretion are associated with variations in circadian clock genes like Clock, Period, and Timeless. Furthermore, circadian biology leads to functional impairments in anxiety, obsessive-compulsive disorder, and substance use disorders. Consequently, chronotherapeutic approaches such as light and wake therapies targeting the resynchronization of the circadian system offer a robust, safe, and innovative potential for treating psychiatric disorders and preventing relapses.
