Sirkadiyen Ritim ve Romatolojik Hastalıklar

Özet

Sirkadiyen ritim, organizmanın fonksiyonlarını 24 saatlik periyotta düzenleyerek bağışıklık sistemi ve inflamatuar süreçler üzerinde kritik bir rol oynamaktadır. Yapılan araştırmalar, bu ritimdeki düzensizliklerin romatolojik hastalıkların patogenezi ve semptom seyri ile doğrudan ilişkili olduğunu göstermektedir. Romatoid Artrit (RA) hastalarında geceleri yetersiz kortizol salgılanması, sabah saatlerinde proinflamatuar sitokinlerin (IL-6) artmasına ve buna bağlı olarak şiddetli eklem ağrısı ile sabah tutukluğuna yol açmaktadır. Bu durum, ilaçların zamanlamasını ayarlayan kronoterapi yaklaşımını ön plana çıkarmış; gece salınımlı prednizon veya gece uygulanan Metotreksat tedavisinin semptomları ve eklem katılıklarını azaltmada daha etkili olduğu kanıtlanmıştır. Psöriatik artritte melatonin düşüklüğü göze çarparken, Ankilozan Spondilitte vertebral tutukluğun sabah ve akşam saatlerinde çift tepe noktası yaptığı saptanmıştır. Sistemik Lupus Eritematozustaki sirkadiyen bozulmanın organ disfonksiyonuna, özellikle de lupus nefritine zemin hazırladığı kabul görmektedir. Fibromiyaljide ise idrardaki melatonin atılım farklılıkları sirkadiyen düzensizliğin bir biyobelirteci olarak öne çıkmakta ve pineal bez hacmi ile hastalık semptomları arasında anlamlı ilişkiler bulunmaktadır. Sonuç olarak, krono-immünolojik veriler romatizmal hastalıkların tanı ve tedavi başarısını optimize etmek için büyük bir potansiyel barındırmaktadır.

The circadian rhythm regulates organismal functions in a 24-hour period, playing a critical role in the immune system and inflammatory processes. Research demonstrates that disruptions in this rhythm are directly associated with the pathogenesis and symptom course of rheumatologic diseases. In Rheumatoid Arthritis (RA) patients, insufficient cortisol secretion at night leads to an increase in proinflamatuar cytokines (IL-6) during morning hours, causing severe joint pain and morning stiffness. This situation highlights the chronotherapy approach, which adjusts drug timing; it has been proven that night-release prednisone or nighttime Methotrexate treatment is more effective in reducing symptoms and joint stiffness. While low melatonin characterizes psoriatic arthritis, vertebral stiffness in Ankylosing Spondylitis shows a double peak in morning and evening hours. It is accepted that circadian disruption in Systemic Lupus Erythematosus paves the way for organ dysfunction, particularly lupus nephritis. In fibromyalgia, differences in urinary melatonin excretion stand out as a biomarker of circadian dysregulation, and significant relationships exist between pineal gland volume and disease symptoms. Consequently, chrono-immunological data holds great potential for optimizing the diagnosis and treatment success of rheumatic diseases.
