Solunum Sistemi Hastalıklarına Sirkadiyen Saat Açısından Bakış

Özet

Merkezi sinir sistemindeki suprakiazmatik çekirdek tarafından yönetilen 24 saatlik biyolojik saatin (sirkadiyen ritim) solunum sistemi hastalıkları, özellikle astım, KOAH, pulmoner fibrozis ve obstrüktif uyku apnesi (OUA) üzerindeki etkileri incelenmektedir. Araştırmalar; solunum semptomlarının, hava yolu fonksiyonlarının ve inflamatuar süreçlerin gece ve gündüz döngülerine göre belirgin değişiklikler gösterdiğini ortaya koymaktadır; örneğin astım krizleri ve KOAH alevlenmelerinin şiddeti gece saatlerinde belirgin şekilde artmaktadır. Bu durum, temel sirkadiyen hormonu olan melatoninin bronşlarda kasılmayı tetiklemesi ve endojen kortikosteroid seviyelerinin diurnal ritimle dalgalanması gibi mekanizmalarla açıklanmaktadır. Ayrıca, sürekli gece vardiyasında çalışan bireylerde orta ve şiddetli astım gelişme riskinin %36 oranında arttığı saptanmıştır. Pulmoner fibroziste doku hasarı ile hücresel asenkronizasyon ilişkili bulunurken, uyku apnesi gibi solunum bozukluklarının da sirkadiyen sistemle karşılıklı bir etkileşim içinde olduğu ve uzun süreli CPAP tedavisiyle bu ritim bozukluklarının hafifletilebildiği belirtilmektedir. Sonuç olarak, hastalıkların patogenezinde sirkadiyen saatin rolünün anlaşılması, ilaçların günün en uygun zamanında verilmesini hedefleyen "kronoterapi" yaklaşımlarının solunum yolu hastalıklarının yönetiminde ve tedavi başarısında kritik bir öneme sahip olduğunu göstermektedir.

The 24-hour biological clock (circadian rhythm), managed by the suprachiasmatic nucleus in the central nervous system, significantly impacts respiratory diseases, particularly asthma, COPD, pulmonary fibrosis, and obstructive sleep apnea (OSA). Research reveals that respiratory symptoms, airway functions, and inflammatory processes vary according to day and night cycles; for instance, the severity of asthma attacks and COPD exacerbations increases markedly during the night hours. This situation is explained by mechanisms such as melatonin, the primary circadian hormone, triggering bronchoconstriction and endogenous corticosteroid levels fluctuating with the diurnal rhythm. Additionally, it has been determined that individuals working permanent night shifts have a 36% increased risk of developing moderate-to-severe asthma. While cellular asynchrony is associated with tissue damage in pulmonary fibrosis, respiratory disorders like sleep apnea interact mutually with the circadian system, and these rhythm disruptions can be alleviated with long-term CPAP therapy. In conclusion, understanding the role of the circadian clock in disease pathogenesis demonstrates that "chronotherapy" approaches, which aim to administer medications at the most optimal time of day, hold critical importance in the management and treatment success of respiratory diseases.
