Özet

Canlıların 24 saatlik sirkadiyen ritminin metabolik süreçler ve kardiyovasküler sistem üzerindeki hayati etkileri büyük önem taşımaktadır. Doğal ışığın beyindeki suprakiazmatik çekirdek tarafından algılanmasıyla başlayan bu ritim; melatonin, kortizol ve adrenalin gibi hormonların salınımını düzenleyerek vücut sıcaklığı ve kan basıncını koordine eder. Vücuttaki sirkadiyen düzensizlikler; diyabet, obezite ve nöropsikiyatrik rahatsızlıkların yanı sıra kardiyovasküler hastalıkları tetikleyebilir. Kalbin kendi lokal saat mekanizmasına sahip olması nedeniyle uyku-uyanıklık döngüsü, kan basıncı ve otonom sinir sistemi aktiviteleri birbirleriyle doğrudan bağlantılıdır. Bu hemodinamik değişimler doğrultusunda akut miyokard enfarktüsü, inme ve ani kardiyak ölüm vakaları, adrenerjik uyarının arttığı sabah saatlerinde belirgin bir zirve yapmaktadır. Atriyal fibrilasyon ve sinüs bradikardisi gibi aritmiler sirkadiyen olarak geceleri yoğunlaşırken; ventriküler taşikardi ve ventriküler fibrilasyon gibi malign aritmiler sabah uyandıktan sonra artış gösterir. Kronobiyoloji biliminin gelişmesiyle birlikte hastaların ilaç tedavilerinin bu döngüye göre kişiselleştirilmesi hedeflenmektedir. Örneğin, beta-blokerlerin ve düşük doz aspirinin gece verilmesinin sabah saatlerindeki kardiyovasküler riskleri azaltmada daha başarılı sonuçlar verdiği gösterilmiştir. Sonuç olarak, sirkadiyen ritim ile aritmiler arasındaki güçlü korelasyonun anlaşılması, koruyucu tıp ve etkili kronoterapi stratejilerinin geliştirilmesine olanak tanımaktadır.

The vital effects of the 24-hour circadian rhythm on metabolic processes and the cardiovascular system carry immense importance. Initiated by the perception of natural light by the suprachiasmatic nucleus in the brain, this rhythm coordinates body temperature and blood pressure by regulating the secretion of hormones like melatonin, cortisol, and adrenaline. Circadian disruptions in the body can trigger cardiovascular diseases, alongside diabetes, obesity, and neuropsychiatric disorders. Since the heart possesses its own local clock mechanism, the sleep-wake cycle, blood pressure, and autonomic nervous system activities are directly interconnected. In line with these hemodynamic changes, cases of acute myocardial infarction, stroke, and sudden cardiac death peak significantly in the morning hours when adrenergic stimulation increases. While arrhythmias such as atrial fibrillation and sinus bradycardia intensify nocturnally, malignant arrhythmias like ventricular tachycardia and ventricular fibrillation show an increase after waking up in the morning. With the advancement of chronobiology, it is aimed to personalize the drug treatments of patients according to this cycle. For instance, administering beta-blockers and low-dose aspirin at bedtime has been shown to yield more successful results in reducing morning cardiovascular risks. Consequently, understanding the strong correlation between circadian rhythm and arrhythmias enables the development of preventive medicine and effective chronotherapy strategies.
