Özet

Kronoterapi ve Kronoteröpatikler başlıklı çalışma, sirkadiyen ritmi ve uyku-uyanıklık döngüsünü düzenleyerek başta duygudurum bozuklukları olmak üzere çeşitli psikiyatrik hastalıkların tedavisini hedefleyen yöntemleri ele almaktadır. Bu doğrultuda makalede parlak ışık tedavisi (fototerapi), karanlık tedavisi, uyku yoksunluğu ve uyku fazı ilerlemesi gibi temel kronoteröpatik yaklaşımlar detaylandırılmıştır. Parlak ışık tedavisi özellikle mevsimsel depresyonda ve uyku fazı bozukluklarında sirkadiyen ritmi senkronize ederek kullanılırken; etki mekanizması kronobiyolojik, serotonin ve otonom sistem hipotezleriyle açıklanmaktadır. Karanlık tedavisi ise manik dönemlerde ve jet-lag durumlarında melatonini stabilize etmek amacıyla uygulanmaktadır. Uyku yoksunluğu tedavisi, unipolar ve bipolar depresif ataklarda ilk 24-36 saat içinde %60'a varan hızlı bir antidepresan etki göstermekle birlikte, etkinin kalıcı olabilmesi için parlak ışık ve uyku fazı ilerlemesiyle birleştirilerek "Üçlü Kronoterapi" şeklinde uygulanmaktadır. Bu yöntemin etki mekanizması nöroplastisite, monoaminerjik sistemler, sitokin seviyeleri ve saat genlerinin sıfırlanması gibi karmaşık biyolojik süreçlerle ilişkilendirilmiştir. Sonuç olarak kronoteröpatik yaklaşımlar; yan etkilerinin az olması, kontrendikasyonlarının sınırlılığı ve ileri evre organ yetmezliği ya da gebelik gibi ilaç kullanımının riskli olduğu hasta gruplarında güvenle uygulanabilmesi sayesinde psikiyatri alanında giderek önem kazanan, farmakoterapiyle kombine edilerek başarı oranı artırılabilecek güçlü alternatifler sunmaktadır.

In this context, the article details fundamental chronotherapeutic approaches such as bright light therapy (phototherapy), dawn/dusk simulation, darkness therapy, sleep deprivation, and sleep phase advance. Bright light therapy is successfully utilized to synchronize circadian rhythms, especially in seasonal depression and sleep phase disorders, and its mechanism of action is explained through chronobiological, serotonin, and autonomic system hypotheses. Darkness therapy is implemented to stabilize melatonin secretion during manic episodes and jet-lag situations. Sleep deprivation therapy demonstrates a rapid antidepressant effect with a response rate of approximately 60% within the first 24-36 hours in unipolar and bipolar depressive episodes; however, to maintain clinical efficacy, it is combined with bright light and sleep phase advance under a protocol known as "Triple Chronotherapy". The mechanism of this therapeutic effect is linked to complex biological processes, including neuroplasticity, monoaminergic systems, alterations in cytokine concentrations, and the resetting of clock genes. Consequently, chronotherapeutic approaches offer significant advantages due to their low side-effect profiles, minimal contraindications, and safety in vulnerable patient populations—such as those with advanced liver or kidney failure and during the peripartum period—where medication use is restricted, presenting powerful alternatives that can be combined with pharmacotherapy to enhance treatment outcomes.
