Çocuklarda Sirkadiyen Ritmin Mekanizması ve Önemi

Özet

Sirkadiyen ritim (SR); uyku-uyanıklık döngüsü, hormon salınımı ve metabolizma gibi fizyolojik süreçleri düzenleyen yaklaşık 24 saatlik biyolojik zamanlama sistemidir. Ritmin ana merkezi hipotalamustaki suprakiazmatik nukleus (SKN) olup, ışık en temel uyarandır; ancak moleküler düzeyde PER ve TIM gibi genetik protein mekanizmaları da bu saati kontrol eder. Melatonin ve kortizol hormonları sirkadiyen ritmin en önemli biyolojik belirteçleridir. Melatonin karanlıkta salınarak uykuyu, bağışıklığı ve kemik yapımını desteklerken; kortizol sabah saatlerinde zirve yaparak gündüz uyanıklığını sağlar. SR gelişimi fetal dönemde anne ritminin etkisiyle başlar; yenidoğanda sıcaklık ritmi hemen görülse de uyku-uyanıklık döngüsü 3-6 aylıkken belirginleşir. Çocuklukta sabahçıl kronotip hâkimken, ergenlikte akşamcıl kronotipe geçiş yaşanır. SR, büyüme hormonu (GH) salınımı ve kemik homeostazı ile doğrudan ilişkilidir. Ritmin bozulması çocuklarda büyüme geriliği, obezite, insülin direnci gibi metabolik sorunların yanı sıra depresyon, DEHB ve anksiyete gibi psikiyatrik bozukluklara yol açabilir. Bu nedenle çocukların uyku saatlerinin ritme uygun düzenlenmesi, okul saatlerinin 08:30 sonrasına alınması, gün içinde egzersiz yapılması ve yatmadan önceki iki saat boyunca mavi ışık maruziyeti ile fiziksel aktivitelerin sınırlandırılması büyük önem taşımaktadır.

Circadian rhythm (CR) is an approximately 24-hour biological timing system that regulates physiological processes such as the sleep-wake cycle, hormone release, and metabolism. The primary center of the rhythm is the suprachiasmatic nucleus (SCN) in the hypothalamus, with light being the most fundamental stimulus; however, genetic protein mechanisms like PER and TIM also control this clock at the molecular level. Melatonin and cortisol hormones are the most critical biological markers of the circadian rhythm. While melatonin is secreted in the dark to support sleep, immunity, and bone formation, cortisol peaks in the early morning hours to provide daytime alertness. CR development begins in the fetal period under the influence of the maternal rhythm; although the temperature rhythm appears immediately in newborns, the sleep-wake cycle becomes distinct at 3-6 months. While the morningness chronotype dominates in childhood, a shift to the eveningness chronotype occurs during adolescence. CR is directly associated with growth hormone (GH) secretion and bone homeostasis. Disruption of the rhythm can lead to metabolic issues such as growth retardation, obesity, and insulin resistance in children, as well as psychiatric disorders like depression, ADHD, and anxiety. Therefore, it is of great importance to arrange children's sleep schedules in accordance with the rhythm, schedule school hours after 08:30, exercise during the day, and strictly limit blue light exposure and physical activities during the two hours before bedtime.
