Diyabet Cerrahisi
Özet
Dünya genelinde obezite ve buna bağlı Tip 2 Diabetes Mellitus (T2DM) prevalansındaki artış epidemik bir boyuta ulaşmıştır. Obezite ile mücadelede ve kilo kaybı sağlamada en etkili yöntem bariatrik cerrahidir. T2DM ve ilişkili metabolik durumların tedavisini hedefleyen bu prosedürler metabolik cerrahisi (MC) olarak tanımlanmaktadır. MC; glisemik kontrolü sağlama, sürekli kilo kaybı ve diyabet komorbiditelerini azaltmada yaşam tarzı değişiklikleri veya tıbbi tedavilere kıyasla üstün etkinlik göstermektedir. Diyabet, mikrovasküler ve makrovasküler komplikasyonlar riski taşıyan, çoklu organ disfonksiyonuna yol açabilen kronik bir hastalıktır. Tedavide sağlıklı yaşam tarzı, diyet ve risk faktörlerinin yönetimi temel basamakları oluştursa da özellikle morbid obez bireylerde uzun vadeli başarı için cerrahi müdahale öne çıkmaktadır. Günümüzde Sleeve Gastrektomi (SG) ve Roux-en-Y Gastrik Bypass (RYGB) en sık uygulanan, minimal invaziv, güvenli ve maliyet-etkin bariatrik yöntemlerdir. Bu cerrahi prosedürler, malabsorbsiyon ve hacim kısıtlamasının yanı sıra endokrin değişiklikler tetikleyerek yüksek oranlarda diyabet remisyonu sağlamakta ve hastaların mortalite oranını bariz şekilde düşürmektedir.
The global rise in obesity and related Type 2 Diabetes Mellitus (T2DM) prevalence has reached epidemic proportions. Bariatric surgery is proven to be the most effective method for managing obesity and achieving sustainable weight loss. Metabolic surgery (MC) describes these procedures primarily used to treat T2DM and associated metabolic conditions. MC demonstrates superior efficacy over lifestyle modifications or medical management in achieving glycemic control, sustained weight loss, and reducing diabetic comorbidities. Diabetes is a chronic disease associated with microvascular and macrovascular complications that can lead to multi-organ dysfunction. Although a healthy lifestyle, diet, and risk factor management form the foundational treatment steps, surgical intervention stands out for long-term success, particularly in morbidly obese individuals. Currently, Sleeve Gastrectomy (SG) and Roux-en-Y Gastrik Bypass (RYGB) are the most commonly performed, minimally invasive, safe, and cost-effective bariatric procedures. These interventions trigger endocrine changes alongside malabsorption and volume restriction, leading to high rates of diabetes remission and significantly reducing patient mortality.
